Objectives of Study：

To verify the safety and efficacy of a key intervention based on cognition combined with frailty screening in preventing falls in hospitalized elderly patients with cardiovascular and cerebrovascular diseases.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥60岁； （2）MORSE>24分； （3）可正确沟通； （4）存在轻度认知功能损伤或衰弱前期； （5）自愿参加本研究。

Inclusion criteria

(1) Age ≥ 60 years old;
(2) MORSE>24 points;
(3) can communicate correctly;
(4) There is mild cognitive impairment or pre-frailty;
(5) Voluntary participation in this study.

排除标准：

（1）长期卧床患者；
（2）NYHA心功能D级；
（3）疾病发作急性期患者。

Exclusion criteria：

(1) Long-term bedridden patients;
(2) NYHA cardiac function class D;
(3) Patients with acute disease onset.
