研究目的：

1、探究Graves眼病患者肠道、眼表菌群改变及菌群参与该疾病发病的可能机制 2、为Graves眼病患者的个体化治疗提供新靶点、新视角  

Objectives of Study：

1.To investigate the possible mechanisms involved in the pathogenesis of Graves' ophthalmopathy with altered gut and ocular surface microbiota and the involvement of the microbiota 2.New targets and perspectives for individualized treatment of patients with Graves' ophthalmopathy

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、年龄18-75岁
2、Graves病入组标准（参考ATA指南）
（1）临床常见的甲亢症状和体征
（2）体格检查和影像学检查提示甲状腺弥漫性肿大
（3）血清TSH水平降低，甲状腺激素水平升高
（4）TRAb阳性
（5）胫前粘液性水肿
（6）眼球突出和其他浸润性眼征
（7）甲状腺摄碘131率升高
其中(1)(2)(3)(4)是诊断必备条件，(5)(6)(7)为诊断辅助条件
3、Graves眼病入组标准（参考EUGOGO指南）
（1）符合GD诊断
（2）有畏光、流泪、眼球疼痛、泪阜肿胀、眼睑/结膜水肿或充血症状，眼部CT或MRI检查支持TAO诊断
（3）视神经功能障碍：视力下降，色觉、视野异常，复视，斜视，无法用其他病变解释
满足第（1）项及（2）、（3）中的任一项

Inclusion criteria

1. Aged 18-75 years
2. Graves' disease entry criteria (referred to ATA guidelines)
(1) Common clinical signs and symptoms of hyperthyroidism
(2) Physical examination and imaging suggested diffuse thyroid enlargement
(3) The serum TSH level decreased and the thyroid hormone level increased
(4) TRAb positive
(5) Pretibial myxoedema
(6) Proptosis and other infiltrative eye signs
(7) Elevated thyroid iodine uptake 131 rate
Among them, (1) (2) (3) (4) are diagnostic requisites, and (5) (6) (7) are diagnostic aids
3. Graves ophthalmopathy enrollment criteria (refer to eugogo guidelines)
(1) Consistent with a Gd diagnosis
(2) Had symptoms of photophobia, tearing, ocular pain, tear Fu swelling, eyelid / conjunctival edema or hyperemia, and ocular CT or MRI examination supported the diagnosis of Tao
(3) Optic nerve dysfunction: decreased visual acuity, color vision, visual field abnormalities, diplopia, strabismus that cannot be explained by other lesions
Satisfying either of items (1) and (2), (3)

排除标准：

有以下情况之一不能纳入本研究：
(1)3个月内使用过抗生素，并持续3天或以上；
(2)3月个内使用任何眼部局部药物者；
(3)有眼部激光治疗及眼部手术史患者；
(4)有眼部外伤病史者；
(5)有配戴角膜接触镜既往史者；
(6)有眼部炎症、翼状胬肉、青光眼、角膜营养不良等眼部疾病者；
(7)除Graves病及其并发症外有其他全身系统疾病（如糖尿病、全身免疫性疾病等）；
(8)中重度肾功能损伤（血肌酐＞2mg/dL或177mmol/L），或肝功能异常（ALT>2倍正常值上限）的患者；中、重度慢性阻塞性肺部疾病；重度高血压患者、脑血管意外患者；
(9)患有严重器质性疾病，如癌症、冠心病、心肌梗塞以及中风等；
(10)患有肺结核和艾滋病等传染病；
(11)发生严重Graves病并发症的患者，如：甲亢危象等；
(12)酗酒（1周饮酒5次以上，平均每次2两白酒或半斤黄酒或5瓶啤酒以上）；
(13)妊娠期以及哺乳期妇女；
(14)肢体残疾，或其他原因引起的生活不能自理、不能清楚地回忆、回答问题者，或有明显的运动障碍；不能保证充足的时间参与本项目的。

Exclusion criteria：

One of the following situations cannot be included in this study:
(1) Had used antibiotics within 3 months and continued for 3 days or more;
(2) Any ocular topical medication use within 3 months;
(3) Patients with a history of ocular laser therapy and ocular surgery;
(4) Those with a history of ocular trauma;
(5) Those with a previous history wearing corneal contact lenses;
(6) Those with ocular diseases such as ocular inflammation, pterygium, glaucoma, and corneal dystrophy;
(7) Systemic diseases other than Graves' disease and its complications (e.g., diabetes, systemic immune diseases, etc.);
(8) Patients with moderate to severe renal impairment (serum creatinine > 2 mg / dL or 177 mmol / L), or abnormal liver function (ALT > 2 times the upper limit of normal; Moderate and severe chronic obstructive pulmonary disease; Patients with severe hypertension, cerebrovascular accident;
(9) Suffering from severe organic diseases such as cancer, coronary heart disease, myocardial infarction as well as stroke, etc;
(10) With infectious diseases such as tuberculosis and AIDS;
(11) Patients who develop severe complications of Graves' disease such as: hyperthyroid crisis, etc;
(12) Binge drinking (5 or more drinks in 1 week, 2 or half kg of yellow rice wine or more than 5 bottles of beer on average);
(13) Gestational as well as lactating women;
Limb disability, or other causes of life that are not self-care, cannot be clearly recalled, answered questions, or have significant movement disorders; Adequate time for participation in this project cannot be guaranteed.
