研究目的：

本研究拟通过观察五行音乐对血管性认知障碍（VCI）患者的认知、情绪、睡眠等方面产生的影响，以及音乐干预前后患者肠道菌群的变化，分析肠道菌群及其相关代谢产物的改变是否参与五行音乐对认知功能的改善作用，从而为VCI患者寻找安全、简便、有效的早期干预手段。  

Objectives of Study：

This study aims to determine the impact of Chinese five-element music on the cognitive function of patients with vascular cognitive impairment. By observing the change of gut microbiota before and after the music-based intervention, identify the role of gut microbiota change in mediating the association of music intervention with the cognitive function change.

药物成份或治疗方案详述：

采用前瞻性、开放标签、终点盲法、随机对照研究设计，纳入就诊于我院神经内科的明确诊断为血管性认知障碍的患者，采用区组随机化方法分为干预组和对照组，对照组仅给予常规治疗，干预组除常规治疗外每天聆听音乐30分钟，每周不少于3次，坚持6个月。音乐采用我国特有的五行音乐，由中医科医师为患者辩证，选择合适的调式歌单。干预前后收集患者的临床资料、神经心理量表评估、影像学资料及肠道菌群测定，比较分析五行音乐对于VCI患者产生的作用，并采用中介效应分析判断肠道菌群改变是否在音乐干预对认知功能的作用过程中存在中介效应。 

Description for medicine or protocol of treatment in detail：

This is a prospective randomized open-label blinded end-point study. Patients with vascular cognitive impairment will be block randomized to the music-listening group or the control group. Patients in the music-listening group will be asked to listen to Chinese five-element music for 3 months, at least 5 days per week, 30 minutes per day. While patients in the control group will take routine medical care only. Cognitive function, mood, sleep quality, cerebral blood flow and gut microbiota will be measured at baseline and after 3 months. Causal mediation analysis will be used to explore the mediation effects.  

纳入标准：

（1）年龄≥50岁
（2）符合2019年中国血管性认知障碍诊治指南中的VCI诊断标准
（3）MoCA≤26分（受教育年限≤12年者加1分）
（4）Hachinski缺血量表完整版≥8分
（5）影像学提示存在脑血管病（脑梗死、脑出血、脑白质病变、微出血、血管周围间隙）
（6）卒中发病后3个月以上

Inclusion criteria

1.age ≥ 50 years
2.fullfilled the diagnostic criteria of vascular cognitive impairment in the 2019 guideline
3.score of MoCA ≤ 26
3.Hachinski's scale ≥ 8
4.cerebral MRI showed lesions of cerebral vascular disease (including cerebral infarction, intracranial hemorrhage, white matter lesion, microbleed, peri-vascular space)
5.at least 3 months after stroke onset

排除标准：

（1）存在其他可能影响认知功能的神经系统疾病，如AD、帕金森病、额颞叶变性、脑外伤、中枢神经系统感染等
（2）存在严重躯体功能障碍，不能配合完成量表测评
（3）存在严重精神疾病
（4）近1月内应用抗生素、益生菌或益生元等
（5）近6周内应用改善智能药物或抗焦虑抑郁药物剂量有所调整
（6）存在磁共振检查禁忌症
（7）病情危重，生命体征不平稳

Exclusion criteria：

1.other neurologic disease may cause cognitive impairment, such as Alzheimer's disease, Parkinson disease, FTD, trauma, infection of central nervous system etc.
2.severe physical dysfunction so that cannot complete the assessment
3.severe psychotic condition
4.used antibiotics or probiotics within 1 month
5.changed dosage of cholinesterase inhibitors or psychotropic medications within the last 6 weeks
6.can not take 3.0T MRI test for any reason
7.serious medical condition required medical treatment and monitoring

由一名专门的科研护士进行随机化分组，采用区组随机化方法分为干预组和对照组，两组比例为1:1，每个区组设为4例。

A research nurse will carry out the randomization, she will not be involved in the enrollment, assessment, or intervention of the participants. Patients enrolled will be block randomized into music-listening group or control group in a 1:1 ratio. There will be 4 cases in each block.
