研究目的：

项目组前期环境流行病学研究发现，母体受孕后的PM2.5暴露可增加人类胚胎停育的发生风险，机制未明。氧化应激及炎症反应是PM2.5致呼吸和心血管疾病的主要机制，是否也介导了PM2.5对胚胎发育的影响有待研究。项目拟依托时间调整土地利用回归模型研究积累、临床一线的病例-对照招募和生物样本采集优势，在预实验基础上开展人体生物标志物检测和人源细胞毒理学实验。测定胚胎停育和正常早孕人群血清及绒毛组织的代表性生物标志物含量，分析PM2.5诱发妊娠早期全身性和母胎界面氧化应激及炎症反应的剂量-效应关系。采集采暖期/非采暖期、交通源/工业源的PM2.5样品，进行人绒毛膜滋养层细胞染毒和干预实验，探索PM2.5及化学组分对细胞氧化应激及炎症反应的影响，筛选出抑制细胞增殖、迁移和侵袭能力的高危化学组分。从母体、组织和细胞水平揭示PM2.5引起人类胚胎停育的效应机制，为采取干预措施预防不良妊娠结局提供科学依据。  

Objectives of Study：

e recently reported that maternal PM2.5 exposures during the post-conceptionperiod could increase the risk of human clinically recognized early pregnancyloss. The mechanisms underlying this association are not yet clear. Oxidativestress and inflammation are the main hypothesized mechanisms of respiratory andcardiovascular diseases caused by PM2.5. Whether these also mediate the effects of PM2.5 on embryonic development remains to be studied. Using our previouslydeveloped temporally-adjusted land use regression PM2.5 models and the advantagesof clinical case-control recruitment and biological sample collection, we intendto conduct human biomarker studies and human cytotoxicity experiments on our basis of pre-experiment. Selected biomarkers in serum and chorionic villous tissue (theprecursor tissue of placenta) will be measured in early pregnancy loss cases andnormal early pregnancy controls. In the epidemiological study, associationsbetween PM2.5 exposure and biomarkers of both systemic and local (at thematernal-fetal interface) oxidative stress and inflammation will be investigated.Further, PM2.5 samples from different periods (heating/non-heating period) anddifferent sources (traffic/industrial source) will be collected. These sampleswill be used in experiments using human chorionic trophoblast cells to assess theeffects of PM2.5 and PM2.5 chemical components on cellular oxidative stress andinflammation, and to screen for especially toxic chemical components that inhibitcellular function. This project will provide insight into the mechanismsunderlying PM2.5-induced human early pregnancy loss at the maternal, tissue andcellular levels. It will also contribute to providing a scientific basis forinterventions to prevent adverse pregnancy outcomes.

纳入标准：

病例组（需同时符合以下情况）：
1. 妊娠天数＜13 周；
2. 平素月经规律(近一年月经周期变化范围为±1 天)；
3. 根据美国妇产科医师学会 2018 年 EPL 临床实践指南，结合患者的病史、体格检查、血人绒毛膜促性腺激素检测及妇科超声检查诊断为 EPL；妇科超声提示宫内妊娠囊，囊内胎芽≥7mm 但未见胎心，或妊娠囊平均直径≥25mm 但未见胎芽，或见卵黄囊后≥11天仍未见有胎心的胎，或间隔≥7 天的两次超声结果对比提示胚胎停止发育者；
4. 在天津市居住 1 年以上，住址位于研究的 10 个辖区内，且近一年住址未发生变动；
5. 自愿参加并签署由天津医科大学第二医院医学伦理委员会审查通过的知情同意书。
对照组（需同时符合以下情况）：
1. 妊娠天数＜13 周且自愿要求终止妊娠者；
2. 平素月经规律(近一年月经周期变化范围为±1 天)；
3. 妇科超声提示“宫内妊娠囊，可见胎芽及胎心”；
4. 孕期无阴道出血；
5. 无不良妊娠史；
6. 在天津市居住 1 年以上，住址位于研究的 10 个辖区内，且近一年住址未发生变动；
7. 自愿参加并签署由天津医科大学第二医院医学伦理委员会审查通过的知情同意书。

Inclusion criteria

Case group (must meet the following conditions at the same time):
1. gestation days < 13 weeks;
2. regular menstruation (the range of menstrual cycle changes in the past year is ± 1 day);
3. according to the 2018 EPL clinical practice guidelines of the American College of Obstetricians and Gynecologists, Medical history, physical examination, blood human chorionic gonadotropin test, and gynecological ultrasound diagnosis of EPL; gynecological ultrasound suggested intrauterine pregnancy sac, fetal buds >= 7mm but no fetal heart, or mean diameter of pregnancy sac >= 25mm but no Fetal buds, or fetuses with no fetal heart >= 11 days after seeing the yolk sac, or comparison of two ultrasound results at intervals of >= 7 days suggest that embryos who have stopped developing
4. have lived in Tianjin for more than 1 year and their addresses are in the 10 jurisdictions studied and the address has not changed in the past year;
5. voluntarily participated in and signed the informed consent form reviewed and approved by the Medical Ethics Committee of the Second Hospital of Tianjin Medical University.
Control group (must meet the following conditions at the same time):
1. those who have gestational days < 13 weeks and voluntarily request termination of pregnancy;
2. regular menstrual regularity (the range of menstrual cycle changes in the past year is ± 1 day);
3. And fetal heart;
4. no vaginal bleeding during pregnancy;
5. no history of adverse pregnancy;
6. resided in Tianjin for more than 1 year, the address is located in the 10 jurisdictions of the study, and the address has not changed in the past year;
7. voluntarily participated and signed by the Tianjin Medical University The informed consent form reviewed and approved by the Medical Ethics Committee of the Second Hospital.

排除标准：

排除标准（符合以下情况之一者）：
1. 存在子宫畸形、子宫肌瘤等影响宫腔形态因素者；
2. 存在内分泌异常或高血压等全身性疾病者；
3. 此次孕期存在生殖道感染性疾病者。

Exclusion criteria：

Exclusion criteria (for those who meet one of the following conditions):
1. those who have uterine malformations, uterine fibroids and other factors affecting the morphology of the uterine cavity;
2. those who have systemic diseases such as endocrine disorders or hypertension;
3. dissatisfied persons.
