研究目的：

1.建立多中心随机对照研究，将我中心多年来研究所得的结果在多中心实践，在多中心、大样本的试验中验证加速康复外科在老年胃癌诊疗中的优势与效果，进一步在镇江地区推广应用； 2.进一步深度验证加速康复外科关键技术的安全性与有效性，探索各核心措施的技术适用准则及注意事项，进而完善老年胃癌加速康复外科诊疗规范； 3. 通过观察胃癌患者术后疼痛评分、焦虑评分、肠道菌群及炎性指标的变化来探讨加速康复外科促进老年胃癌患者术后器官功能恢复的机制，为胃癌加速康复外科的发展奠定病理生理基础。  

Objectives of Study：

1. To establish a multi-centre randomised controlled study to validate the advantages and effectiveness of accelerated rehabilitation surgery in the treatment of geriatric gastric cancer in a multi-centre, large sample trial, and further promote its application in the Zhenjiang region by putting into practice the results of our research over the years. 2. to further validate the safety and effectiveness of key techniques of accelerated rehabilitation surgery, explore the guidelines and considerations for the application of each core measure, and then improve the standard of treatment for accelerated rehabilitation surgery for geriatric gastric cancer. 3. to explore the mechanism of accelerated rehabilitation surgery to promote postoperative organ function recovery in elderly gastric cancer patients by observing the changes in postoperative pain scores, anxiety scores, intestinal flora and inflammatory indexes, and to lay the pathophysiological foundation for the development of accelerated rehabilitation surgery for gastric cancer.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥65岁；
（2）术前未接受过化疗等抗肿瘤治疗；
（3）均接受择期D2手术治疗；
（4）患者及家属知情同意。

Inclusion criteria

(1) Age ≥ 65 years.
(2) No preoperative anti-tumour treatment such as chemotherapy.
(3) All treated with elective D2 surgery.
(4) Informed consent of the patient and family.

排除标准：

（1）经医师判定认为不适合参加本试验者；
（2）胃癌患者年龄＜65岁；
（3）缺血性心脏病、脑血管病、外周血管疾病的患者，心功能＞II级（NYHA）的患者，近期接受冠状动脉旁路移植术CABG手术患者，重度高血压患者（收缩压≥180mmHg或舒张压≥110mmHg）；
（4）伴有严重感染、呼吸功能不全、凝血功能障碍、严重肝、肾功能不全等严重器官功能障碍的患者（Child-Pugh≥10分；肌酐清除率＜25ml/min）；
（5）阿片类、非甾体类药物、头孢类抗生素等住院期间常用药物过敏的患者；
（6）伴有完全性胃肠梗阻等并发症的患者，急诊或姑息性手术者；
（7）老年胃癌患者手术需要联合切除脾或胰腺的患者；
（8）存在恶液质、严重营养不良的患者（白蛋白≤30g/L、半年体重减轻＞10%或SGA营养状况评测C级、BMI＜18、Hb＜70g/L）；
（9）无法完成加速康复外科程序或有明显加速康复外科禁忌症的患者；
（10）重度水肿、感染、末梢血液循环障碍患者；
（11）妊娠、哺乳期妇女，试验后1个月内有妊娠计划的受试者（亦包括男性受试者）；
（12）试验前3个月内参加其它临床试验；
（13）直接参与试验的申办者或研究者或他们的家庭成员；
（14）患者或患者家属不同意参加快速康复手术治疗。

Exclusion criteria：

(1) Those patients who are judged by the doctor to be unfit for the test;
(2) The age of patients with gastric cancer is less than 65 years;
(3) Patients with ischemic heart disease, cerebrovascular disease and peripheral vascular disease, patients with cardiac function>Level II (NYHA), patients who have recently undergone coronary-artery-bypass-grafting(CABG) surgery, and patients with severe hypertension (systolic blood pressure ≥ 180mmHg or diastolic blood pressure ≥ 110mmHg);
(4) Patients with severe organ dysfunction such as severe infection, respiratory insufficiency, coagulation dysfunction, severe liver and renal insufficiency (Child-Pugh ≥ 10 points; creatinine clearance<25ml/min);
(5) Patients with opioid, non-steroidal drugs, cephalosporins and other commonly used drug allergy during hospitalization;
(6) Patients with complete gastrointestinal obstruction and other complications, emergency or palliative surgery;
(7) Patients with gastric cancer in the elderly who need combined splenectomy or pancreatectomy;
(8) Patients with cachexia and severe malnutrition (albumin ≤ 30g/L, half-year weight loss>10% or SGA nutritional status evaluation grade C, BMI<18, Hb<70g/L);
(9) Patients who are unable to complete accelerated rehabilitation surgery procedures or have obvious contraindications to accelerated rehabilitation surgery;
(10) Patients with severe edema, infection and peripheral blood circulation disorder;
(11) Pregnant and lactating women, subjects with pregnancy plan within 1 month after the trial (also including male subjects);
(12) Participate in other clinical trials within 3 months before the trial;
(13) The sponsor or researcher directly participating in the trial or their family members;
(14) The patient or his family members do not agree to participate in rapid rehabilitation surgery.
