研究目的:

1.分析肠道菌群及其代谢产物在不同亚型MAFLD患者中的作用。 2.探索肠道菌群及其代谢产物中有望成为MAFLD的非侵入性诊断标志物,用于疾病早期诊断和对疾病严重程度的评估。  

Objectives of Study:

1. To analyze the role of intestinal flora and its metabolites in patients with different subtypes of MAFLD. 2. To explore the intestinal flora and its metabolites, which are expected to be non-invasive diagnostic markers of MAFLD, for early diagnosis of disease and assessment of disease severity.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

(1)自愿签署知情同意书;
(2)年龄 18 - 70 周岁,性别不限;
(3)受试者愿意严格按照采样操作步骤采集和寄送样本;
(4)生命体征、体格检查、血常规、血生化、尿常规均处于正常值范围内或出现异常但研究者认为无意义的异常,同时体重指数为19.0-23.0kg/m2(含界值,计算公式=体重/身高 2),腰围<90cm(男性)或80cm(女性);
(5)明确诊断为 MAFLD,应满足以下两个条件:经影像学、血液生物标记物/NAS 活动性积分(NAS病理评分系统)或肝组织学诊断,确诊肝脏脂肪变性;合并以下三项条件之一:
①超重/肥胖:BMI≥23kg/m2;
②2型糖尿病病史或空腹血糖≥7.0mmol/L;
③代谢功能障碍,存在至少以下两项代谢异常风险因素者:
1)腰围:亚洲人男性和女性分别≥90和80cm;
2)血压:≥130/85mmHg 或接受降血压药物治疗;
3)血液甘油三酯:≥150mg/dL(≥1.70mmol/L)或接受降血脂药物治疗;
4)血浆高密度脂蛋白胆固醇:男性<40mg/dL(<1.0 mmol/L)和女性<50mg/dL(<1.3mmol/L)或接受调脂药物治疗;
5)糖尿病前期:空腹血糖 100-125mg/dL(5.6-6.9mmol/L)或餐后 2 小时血糖140-199mg/dL (7.8-11.0mmol/L)或糖化血红蛋白为 5.7-6.4%(39-47mmol/L);
6)稳态模型评估胰岛素抵抗指数:≥2.5;
7)血液超敏 C 反应蛋白:>2mg/L;

Inclusion criteria

(1) Voluntary signing of informed consent;
(2) Age 18-70 years old, gender unlimited;
(3) The subject is willing to collect and send samples in strict accordance with the sampling procedures;
(4) Vital signs, physical examination, blood routine, blood biochemistry and urine routine were all within the normal range or abnormal but considered meaningless by researchers; body mass index (BMI) was 19.0-23.0kg/m2 (including the boundary value, calculated by formula = weight/height 2); waist circumference <90cm (male) or 80cm (female);
(5) For a definite diagnosis of MAFLD, the following two conditions should be met: diagnosis of hepatic steatosis by imaging, blood biomarker /NAS activity score (NAS pathology score system) or hepatic histology; Merge one of the following three conditions:
① Overweight/obesity: BMI≥23kg/m2;
② History of type 2 diabetes mellitus or fasting blood glucose ≥7.0mmol/L;
③ Metabolic dysfunction, with at least the following two risk factors for metabolic abnormalities:
1) Waist circumference: Asian men and women ≥90 and 80cm, respectively;
2) Blood pressure: ≥130/85mmHg or receiving blood pressure lowering drugs;
3) Blood triglycerides: ≥150mg/dL (≥1.70mmol/L) or receive treatment with lipid-lowering drugs;
4) Plasma HDL cholesterol: <40mg/dL (<1.0 mmol/L) for men and <50mg/dL (<1.3mmol/L) for women or receiving lipid-regulating drugs;
5) Pre-diabetes: fasting blood glucose of 100-125mg/dL (5.6-6.9mmol/L) or blood glucose of 140-199mg/dL (7.8-11.0mmol/L) 2 hours after meal or glycosylated hemoglobin of 5.7-6.4% (39-47mmol/L);
6) Insulin resistance index assessed by homeostasis model: ≥2.5;
7) Blood hypersensitive C-reactive protein: >2mg/L;

排除标准:

(1)伴有可能干扰研究数据的慢性疾病,包括但不限于慢性感染、任何肠道疾病或症状(包括但不限于便秘、腹泻等症状及结肠炎、克罗恩病、肠梗阻、肠道肿瘤等疾病)等;
(2)筛选前1个月内发生过可能干扰研究数据的严重急性疾病;
(3)肝硬化;
(4)研究者认为不适合进入研究的受试者,如精神不佳、依从性不好等。

Exclusion criteria:

(1) Accompanied by chronic diseases that may interfere with the study data, including but not limited to chronic infections, any intestinal diseases or symptoms (including but not limited to constipation, diarrhea and other symptoms, colitis, Crohn's disease, intestinal obstruction, intestinal tumors, etc.);
(2) Severe acute illness occurred within 1 month prior to screening that may interfere with the study data;
(3) Cirrhosis of the liver;
(4) Subjects deemed unsuitable for the study by the researcher, such as poor spirits and poor compliance.
