研究目的：

对比剂急性肾损伤 (Contrast-induced acute kidney injure, CI-AKI)是医源性急性肾损伤的第三大原因，与不良心血管事件及死亡转归密切相关。增强型体外反搏治疗(Enhanced External Counterpulsation, EECP)可以改善肾灌注、增加肾血流，但能否减少CI-AKI的发生仍不清楚。本项目拟开展一项单中心、前瞻性、随机对照研究，观察短疗程EECP治疗对冠脉造影术后对比剂急性肾损伤发生率的影响，以验证短疗程EECP治疗减少冠脉造影术后CI-AKI发生率的科学假说，为EECP预防冠脉造影术后发生CI-AKI提供循证医学证据，也为CI-AKI的防治提供新的方法。  

Objectives of Study：

Contrast-induced acute kidney injure is the third leading cause of iatrogenic acute kidney injury, outcome and adverse cardiovascular events and death are closely related. Enhanced external counterpulsation treatment can improve the renal perfusion, increase renal blood flow, but can reduce the occurrence of CI - AKI are still unclear. This project intends to carry out a single-center, prospective, randomized, controlled studies, to observe the short course EECP therapy on the incidence of acute kidney injury after coronary angiography contrast agent, to verify that the short course EECP therapy to reduce the incidence of CI - AKI after coronary angiography of scientific hypothesis, as EECP prevent coronary angiography after CI - AKI provides evidence of evidence-based medicine, also provide a new method for CI - AKI prevention.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1: 年龄≥18岁

2: 有择期行冠脉造影术检查适应证

Inclusion criteria

1: The age of 18 years old or more

2: Undergoing elective coronary angiography examination indication

排除标准：

1: EECP治疗禁忌证：急性心力衰竭；伴血流动力学不稳定的恶性心律失常；重度主动脉瓣关闭不全；活动性静脉炎、静脉血栓形成；严重下肢血管闭塞性疾病；各种出血性疾病或出血倾向；夹层动脉瘤；反搏位置感染；恶性肿瘤；妊娠期或哺乳期女性。

2: 冠脉造影术禁忌证：对比造影剂过敏试验阳性及碘过敏；易过敏体质，如支气管哮喘、严重过敏性皮炎、食物过敏史等；严重肝功能不全；甲状腺功能亢进症；妊娠期或哺乳期女性；既往发生过CI-AKI。

3: 冠脉造影后继续行介入治疗或相关检查，如：PCI、PTCA、OCT、IVUS、FFr等。

4: 冠脉造影术前1个月内有肾毒性药物或食物服用史。

5: 冠脉造影术前1周内曾接受过血管造影术（包含增强CT等）。

6: 已接受肾脏替代治疗，如血液透析、腹膜透析及肾脏移植等。

7: 同期参加其他临床研究。

Exclusion criteria：

1: EECP contraindications: acute heart failure

2: With hemodynamic instability of malignant arrhythmia

3: Severe aortic valve closure is not complete

4: Active phlebitis, venous thrombosis

5: Severe lower limb vascular occlusive diseases

6: All sorts of hemorrhagic disease or bleeding tendency

7: Dissecting aneurysm

8: Counterpulsation position infection

9: Malignant tumor

10: During pregnancy or breastfeeding women.

11: Coronary angiography contraindicated: contrast contrast sensitivity test positive and iodine allergy

12: Allergy sufferer, such as bronchial asthma, severe atopic dermatitis, food allergies, etc.

13: Severe hepatic insufficiency

14: Hyperthyroidism

15: Women during pregnancy or lactation

16: Always happened in CI - AKI.

17: Continue after coronary angiography interventional treatment or related inspection: PCI, PTCA, OCT, IVUS, FFr, etc.

18: Within a month before the coronary angiography has a history of renal toxicity drugs or food.

19: Within 1 week before coronary angiography has received angiography (including enhanced CT, etc.).

20: Have accepted renal replacement therapy, such as hemodialysis, peritoneal dialysis and kidney transplants.

21: At the same time to participate in other clinical studies.
