研究目的：

脑死亡发生后由于快速明显的激素水平下降，会导致循环衰竭和脏器灌注障碍，影响脏器功能，造成供体器官不必要的浪费，而此时进行激素替代治疗可以减少脏器损伤。我们基于我国国情的器官捐献临床实践和中国人病理生理特性，提出“甲状腺素+甲泼尼龙+血管加压素+胰岛素”的四联用药激素包方案，通过此激素替代治疗方案的实施，评估改方法是否可以改善血流动力学状态，增加脏器灌注，从而提高供体脏器维护成功率。  

Objectives of Study：

The rapid and obvious decrease of hormone level after brain death will lead to circulatory failure and organ perfusion disorder, which will affect organ function and cause unnecessary waste of donor organs. At this time, hormone replacement therapy can reduce organ damage. Organ donation clinical practice based on the situation of our country and we Chinese pathological physiological characteristics, put forward "prednisolone + + armor thyroid hormone vasopressin + insulin" quadruple medicine hormone package scheme, through the implementation of the hormone replacement therapy, whether change assessment method can improve the hemodynamic status, increase viscera perfusion, so as to improve the success rate of donor organs to maintain.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

18岁<年龄≤60岁，达到脑死亡标准和循环不稳定。按照中国脑死亡判定和技术规范，制定本研究的脑死亡标准：脑部病变明确、深昏迷(格拉斯哥昏迷评分3分)、无自主呼吸、脑干反射消失、脑电静息状和脑循环终止。

Inclusion criteria

18 years old < age ≤60 years old, brain death criteria and circulatory instability. According to the Chinese brain death criteria and technical specifications, the criteria for brain death in this study were defined as follows: definite brain lesions, deep coma (Glasgow Coma scale 3), absence of spontaneous breathing, absence of brainstem reflexes, resting EEG and termination of cerebral circulation.

排除标准：

不符合器官捐献标准，经过供体维护最终无法达到捐献标准（捐献标准由医院OPO办公室组织专家依据中国器官捐献即获取指南共同评估决定），存在血流感染，其他原因未完成本研究者。

Exclusion criteria：

It does not meet the organ donation standards, and finally fails to meet the donation standards after donor maintenance (the donation standards are jointly evaluated and determined by experts organized by the OPO office of the hospital according to the guidelines for organ donation and acquisition in China), there are bloodstream infections, and other reasons are not complete for researchers.
