研究目的：

1.评价微调结合芒针透刺等整合康复方案治疗腰椎间盘突出症临床结局。 2.证实方案有助于腰椎间盘突出症的减压减张、强筋束骨，重建腰部稳定性。 3.解析整合康复策略可抑制LDH中枢敏化、促进免疫应答和脑区功能重组，丰富“筋骨失衡，以筋为先”理论的内涵，为LDH防治提供新的科学依据。  

Objectives of Study：

1. To evaluate the clinical outcomes of integrated rehabilitation programs such as fine-tuning combined with acupuncture for lumbar disc herniation. 2. It is confirmed that the program is helpful for decompression and reduction of lumbar intervertebral disc herniation, strengthening muscles and bones, and rebuilding the stability of the waist. 3. Analysis of integrated rehabilitation strategies can inhibit the central sensitization of LDH, promote immune response and functional reorganization of brain regions, enrich the connotation of the theory of ''muscle-bone imbalance, tendons first'', and provide a new scientific basis for the prevention and treatment of LDH.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 有腰部外伤、慢性劳损或受寒湿史，大部分患者在发病前有慢性腰痛史。
2. 腰痛可伴有臀部及下肢放射麻木，腹压增加(如咳嗽、喷嚏)时疼痛加重。
3. 脊柱侧弯，腰椎生理弧度消失或反弓，病变部位椎旁有压痛，可向下肢放射，腰椎活动明显受限。
4. 下肢受累神经支配区有感觉过敏或迟钝，病程长者可出现肌肉萎缩。直腿抬高或加强试验阳性，膝、跟腱反射减弱或消失，拇趾背伸或跖屈肌力减弱。
5. X射线摄片检查可显示：脊柱侧弯，腰生理前凸消失，病变椎间盘可能变窄，相邻边缘有骨赘增生。CT检查可显示：椎间盘突出的部位及程度，判别腰椎椎管狭窄状况；MRI检查可显示：椎间盘退变或在纤维环后方可见高信号区、髓核突出、硬膜囊及神经根受压，并与椎管内肿瘤鉴别，影像检查有助于明确诊断及选择合理治疗方法。
6. 年龄在20-60岁之间，男女不限。
7. 签署知情同意书，愿意接受康复治疗方案，临床诊疗过程符合医学伦理学标准。

Inclusion criteria

1. There is a history of lumbar trauma, chronic strain or exposure to cold and dampness. Most patients have a history of chronic low back pain before the onset of the disease.
2. Low back pain may be accompanied by radiating numbness of the buttocks and lower limbs, and the pain will increase when the abdominal pressure increases (such as coughing and sneezing).
3. Scoliosis, the physiological curvature of the lumbar spine disappears or is reversed, there is tenderness in the paravertebral part of the lesion, which can radiate to the lower limbs, and the lumbar spine movement is obviously limited.
4. Hyperesthesia or sluggishness in the nerve innervation area of the lower extremity, and muscle atrophy may occur in the elderly. The straight leg raising or strengthening test was positive, the knee and Achilles tendon reflexes were weakened or disappeared, and the strength of the hallux dorsiflexor or plantar flexor was weakened.
5. X-ray examination can show: scoliosis, disappearance of lumbar lordosis, possible narrowing of the diseased intervertebral disc, and osteophyte hyperplasia on the adjacent edge. CT examination can display: the location and degree of intervertebral disc herniation, and determine the condition of lumbar spinal stenosis; MRI examination can show: intervertebral disc degeneration or high signal area behind the annulus fibrosus, nucleus pulposus protrusion, dural sac and nerve root compression, in addition, it can be differentiated from intraspinal tumors, and imaging examinations are helpful to confirm the diagnosis and select reasonable treatment methods.
6. Aged between 20-60 years, no gender restrictions.
7. Sign the informed consent form, be willing to accept the rehabilitation treatment plan, and the clinical diagnosis and treatment process conforms to medical ethics standards.

排除标准：

1. 腰椎Ⅱ度以上滑脱伴腰椎峡部裂或明显腰椎不稳患者。
2. 明显腰椎椎管狭窄、黄韧带肥厚或钙化患者。
3. 伴有大小便异常、鞍区麻木等马尾神经损伤症状患者。
4. 患者有肿瘤、结核、血液病、感染等疾病及相关药物过敏史。
5. 患者下肢运动能力缺损，明显肌张力增高，腱反射亢进，踝阵挛等反映上运动神经元或脊髓病变表现。
6. 患者步态出现萎软、慌张、摇摆、或共济失调等可疑中枢、脊髓或骨骼肌肉病变异常。
7. 患者合并严重的心脑血管、肝、肾等严重危及生命的原发性疾病、控制不佳的糖尿病及精神病患者。
8. 患者体质较弱或伴有严重骨质疏松。
9. 妊娠、哺乳期和正在行经期的妇女。

Exclusion criteria：

1. Patients with lumbar spondylolisthesis of degree II and above with lumbar spondylolisthesis or obvious lumbar instability.
2. Patients with obvious lumbar spinal stenosis, ligamentum flavum hypertrophy or calcification.
3. Patients with symptoms of cauda equina nerve injury such as abnormal urination and numbness in the saddle area.
4. The patient has a history of tumor, tuberculosis, blood disease, infection and other diseases and related drug allergies.
5. The patient's lower extremity motor ability deficit, obvious increased muscle tone, tendon hyperreflexia, ankle clonus, etc. reflect the manifestations of upper motor neuron or spinal cord lesions.
6. The patient's gait appears weak, flustered, swaying, or ataxia and other suspicious central, spinal cord or skeletal muscle lesions.
7. Patients with serious cardiovascular, cerebrovascular, liver, kidney and other serious life-threatening primary diseases, poorly controlled diabetes and mental illness.
8. The patient's constitution is weak or accompanied by severe osteoporosis.
9. Pregnancy, lactation and menstruating.
