研究目的:

本研究通过一系列临床和实验室指标比较三种不同的切口设计(封套切口、角形切口、改良角形切口),旨在研究阻生下颌第三磨牙预防性拔除对邻近第二磨牙牙周状况的影响。为了进一步其作用机理,通过 Illumina Mi Seq高通量测序技术对邻近第二磨牙龈下牙菌斑微生物进行微生物多样性分析,比较其菌斑微生物多样性及群落结构的差异。  

Objectives of Study:

This study compared three different incision designs (envelope incision, triangular incision, and modified triangular incision) with a series of clinical and laboratory indicators to investigate the effects of preventive extraction of impacted lower third molars on periodontal status of adjacent second molars. In order to further understand its mechanism, Illumina Mi Seq high-throughput sequencing technology was used to analyze the microbial diversity of adjacent subgingival plaque microbes, and to compare the differences of microbial diversity and community structure.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

①所有拔除的ILTM均为完全阻生(完全骨内)或粘膜下(完全粘膜下),与ASM 位置关系密切;②根据全景X线片,选择技术难度相似的BII类近中ILTM患者。ASM远中牙槽嵴距牙釉质-牙槽骨交界处4-5mm;③ILTM及ASM无任何局部炎症或其他病变;④ASM位于牙弓内,呈垂直位,ASM长轴与ILTM牙冠的夹角为101°~169°;⑤ILTM拔除手术时间小于30min; ⑥ILTM拔除后创面严密缝合; ⑦患者自愿参加临床检查和龈下菌斑取样,依从性好,定期随访。

Inclusion criteria

(1) all ILTM removed were completely impacted (entirely within the bone) or submucosal (completely below the mucous membrane), had a close positional relationship with the adjacent M2 and no contact with the oral cavity. (2) Based on panoramic radiographs, patients with mesioangular ILTM categorized as BII class were selected with similar technical difficult. The distal alveolar crest of the adjacent second molar is 4-5mm away from the enamel-cemental junction. (3) ILTM and the adjacent M2 not have any local inflammation or other pathosis. (4) M2 was in the upright position, and the angle between the long axis of adjacent M2 and the crown of ILTM was 101° ~ 169°. (5) The ILTM operation time was less than 30 minutes; (6) The wound is tightly closed after ILTM extraction; (7) Patients voluntarily participated in clinical examination and subgingival plaque sampling with good compliance and regular follow-up.

排除标准:

①伴有颌骨骨折、慢性全身性疾病(糖尿病、心血管疾病等)的患者、吸烟习惯;②妊娠期或哺乳期妇女;③慢性牙周病患者;④近90天内服用类固醇或非甾体类消炎药、益生菌、抗生素、双磷酸盐的患者;⑤颌骨囊肿或肿瘤患者。

Exclusion criteria:

(1) patients with concomitant jaw fractures, chronic systemic illness (diabetes mellitus, cardiovascular diseases, and so on). (2) the habit of smoking. (3)pregnancy during the extraction. (4) patients with chronic periodontal disease. (5) patients on steroids or non-steroidal anti-inflammatory drugs, probiotics, antibiotics and bisphosphonates within the past 90 days. (6) patients with a cyst or tumor of the jaw.
