研究目的：

在台灣，每年估計有52,000人發生創傷性腦損傷。創傷性腦損傷可導致嚴重且終生的認知、身體和行為障礙，需要長期獲得醫療和失能服務。研究顯示復健可以改善創傷性腦損傷病人的功能。急性後期照護的主要目標在於功能性恢復和減少失能，並使病人有可能返回家庭和社會，減輕健保醫療、家庭與社會負擔。台灣全民健康保險自2017年起提出一項針對外傷性神經損傷患者的急性後期照護計劃，讓有復健潛能的病患在區域或社區醫院接受急性後期照護與復健。台中醫院為健保署急性後期照護合格醫院，本研究將利用107年和111年在台中醫院接受急性後期照護計畫創傷性腦損傷的照護紀錄次級資料庫，進行回溯性分析，來探討急性後期照護計畫是否能改善創傷性腦損傷病人的功能及生活品質。  

Objectives of Study：

In Taiwan, an estimated 52,000 traumatic brain injury occur annually. TBI can result in substantial and lifelong cognitive, physical, and behavioral impairments that require long-term access to health care and disability services. Acute rehabilitation has proven to improve functional outcome after TBI. Post-acute care (PAC) to maximize their functional recovery, reduce disability, and make it possible for them to return to their home and community. PAC can be provided in different settings. Taiwans National Health Insurance (NHI) proposed a PAC program since 2017 for traumatic neurological injury patients, which allowed the patients with the potential for functional improvement to receive PAC rehabilitation in regional or community hospitals. The purpose of this study was to explore the initial achievements and clinical impact of this program in Taiwan. This was a retrospective cohort study that mainly analyzed the basic hospitalization data and the scores for function and quality of life form 2018-2022 de-identified database for traumatic brain injury patients in this PAC program in Taichung hospital.

纳入标准：

納入條件:年滿20歲，因創傷性腦損傷造成失能且發病60天內之病人，功能狀況具有顯著且持續之中度功能障礙(巴氏量表40至70分) ，醫療狀況穩定，於參與本計畫之醫事機構住院，經醫療團隊判斷具積極復健潛能者。

Inclusion criteria

The inclusion criteria of the PAC program included : 1)patients age was greater than or equal to 20 years, 2)within 60 days of traumatic brain injury (ICD10:S06.0-A), 3)with significant and persistent moderate function impairment defined as Barthel index scores range from 40-70, 4) in stable medication condition, and 5)with the potential for functional recovery after rehabilitation

排除标准：

排除條件:嚴重精神疾病，長期呼吸依賴，末期疾病，長期臥床身體功能無法回復，癌症仍需後續住院治療，合併其他之多重創傷仍未穩定者, 如仍有不穩定之骨折，脊髓損傷。

Exclusion criteria：

The exclusion criteria were severe mental illness, long-term respiratory dependence, terminal diseases, long-term bedridden, cancer requiring medical treatment, combined with other multiple traumas that are still unstable, and spinal cord injury.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究將經由台中醫院院本部同意並在台中醫院醫學倫理委員會督考，在確保不影響病人隱私及權利狀下，取得「台中醫院急性後期照護計畫資料庫」，用去病人辨識的次級資料庫，來進行分析。「台中醫院急性後期照護計畫資料庫」內容包含加入上述健保署急性後期照護計畫個案的性別、年齡、住院疾病、住院時間、入出院及住院中的成效評量分數和鼻胃管、尿管是否拔除紀錄及出院後轉歸。該資料庫出自台中醫院個案管理系統和病歷，經由資訊中心去病人辨識後建立及後續管理。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

This study will be approved by Taichung Hospital Headquarters and supervised by the Medical Ethics Committee of Taichung Hospital. To ensure that the patient's privacy and rights are not affected, the "Taichung Hospital Post Acute Care Program Database" will be obtained for secondary patient identification database for analysis. Taichung Hospital’s post-acute care program database contains the gender, age, in-hospital disease, length of hospital stay, admission and discharge and in-hospital outcome evaluation scores and post-discharge outcomes of the cases that joined the above-mentioned National Health Insurance Department’s acute post-care program. The database comes from the case management system and medical records of Taichung Hospital, and is established and followed up by the information center after patient identification.
