研究目的:

糖尿病足溃疡(Diabetic foot ulcers,DFU)是糖尿病患者最常见的严重下肢并发症之一,糖尿病患者一生中发生足部溃疡的风险约为19~34%。即使溃疡痊愈,复发也非常常见。糖尿病外周血管疾病(Diabetic peripheral vascular disease,PVD)引起的组织缺血缺氧和糖尿病周围神经病变(Diabetic peripheral neuropathy,DPN)引起的下肢感觉异常是糖尿病足溃疡难以愈合的重要影响因素。胫骨横向骨搬移(Tibial cortex transverse transport,TTT)技术是治疗糖尿病足溃疡等严重慢性肢体缺血性疾病的新兴外科手术,在下肢缺血性疾病中已得到较多应用。研究表明,TTT不仅促进严重和顽固的糖尿病足溃疡的愈合,减少溃疡的复发,同时也促进了非糖尿病性顽固性下肢溃疡的愈合,与常规手术相比,明显缩短了创面愈合时间。糖尿病患者的足部麻木或疼痛可以通过腓肠神经、腓神经和胫神经的双重挤压来解释。因此,通过手术减压来逆转第二次挤压或病理可能会使得周围神经病变症状得到改善。由此发展了周围神经松解术。神经松解术治疗糖尿病患者因叠加周围神经卡压综合征引起的症状已被报道可获得良好的临床结果,可改善DPN患者的疼痛及感觉功能障碍,改善下肢微循环,使得下肢血管阻力降低,血流量增加,改善神经传导功能,同时可减少足部溃疡发生及复发。TTT技术和神经松解术均被报道对于DFU患者的溃疡愈合及降低DFU的截肢率具有积极作用,然而,目前尚无关于TTT技术和神经松解术联合应用在糖尿病足结局改善的相关研究报道。因此,本研究拟通过临床征集Wagner3~4级糖尿病足溃疡患者,采用TTT技术联合神经松解术对患者患肢进行处理,观察其对Wagner3~4级糖足创面治疗的临床效果,探索一种能够有效治疗难治性糖尿病足的新方法。  

Objectives of Study:

Diabetes foot ulcers (DFU) is one of the most common severe lower limb complications in patients with diabetes. The risk of foot ulcers in patients with diabetes is about 19~34% in their lifetime. Even if the ulcer is cured, recurrence is very common. Tissue ischemia and hypoxia caused by diabetes peripheral vascular disease (PVD) and paresthesia of lower limbs caused by diabetes peripheral neuropathy (DPN) are important influencing factors for the difficult healing of diabetes foot ulcer. Tibial cortex transverse transport (TTT) is a new surgical technique for the treatment of severe chronic limb ischemic diseases such as diabetes foot ulcer. It has been widely used in lower limb ischemic diseases. Studies have shown that TTT can not only promote the healing of severe and refractory diabetes foot ulcer and reduce the recurrence of ulcer, but also promote the healing of non diabetes refractory lower limb ulcer. Compared with conventional surgery, TTT can significantly shorten the wound healing time. The numbness or pain of feet in patients with diabetes can be explained by the double compression of sural nerve, peroneal nerve and tibial nerve. Therefore, surgical decompression to reverse the second crush or pathology may improve the symptoms of peripheral neuropathy. Therefore, peripheral nerve lysis was developed. Neurolysis for the treatment of symptoms caused by superimposed peripheral nerve entrapment syndrome in patients with diabetes has been reported to obtain good clinical results. It can improve pain and sensory dysfunction in patients with DPN, improve microcirculation of lower limbs, reduce vascular resistance of lower limbs, increase blood flow, improve nerve conduction function, and reduce the occurrence and recurrence of foot ulcer. Both TTT and neurolysis have been reported to have a positive effect on ulcer healing and reducing the amputation rate of DFU in patients with DFU. However, there is no relevant research report on the combined application of TTT and neurolysis in improving the outcome of diabetes foot. Therefore, this study intends to collect Wagner grade 3~4 diabetes foot ulcer patients, use TTT technology combined with neurolysis to treat the affected limbs, observe its clinical effect on Wagner grade 3~4 diabetic foot wound, and explore a new method that can effectively treat refractory diabetes foot.

纳入标准:

①18-70周岁,男女不限;②符合2型糖尿病诊断标准(根据1999年WHO糖尿病诊断标准):糖尿病症状加任意时间血浆葡萄糖≥11.1mmol/L(200mg/dl),或空腹血浆葡萄糖水平(FPG)≥7.0mmol/L(126mg/dl),或口服葡萄糖耐量试验(OGTT)中2 小时血糖值(2hPG)≥11.1mmol/L(200mg/dl);③分级为Wagner3~4级糖尿病足的患者:3级 深部溃疡伴脓肿、骨髓炎或败血症;4级 局部坏疽(趾、足跟或前足背),特征为缺血性坏疽,通常合并神经病变;④保肢意愿强烈,患肢腘动脉条件好;⑤未合并严重并发症和其他严重疾病,无绝对手术禁忌者;⑥患者知情并自愿参与本研究。

Inclusion criteria

① 18-70 years old, male or female; ② Meet the diagnostic criteria for type 2 diabetes (according to the WHO diagnostic criteria for diabetes in 1999): diabetes symptoms plus plasma glucose ≥ 11.1mmol/l (200mg / dl) at any time, or fasting plasma glucose level (FPG) ≥ 7.0mmol/l (126mg / dl), or 2-hour blood glucose value (2hPG) in oral glucose tolerance test (OGTT) ≥ 11.1mmol/l (200mg / dl); ③ Patients with Wagner grade 3-4 diabetes foot: Grade 3 deep ulcer with abscess, osteomyelitis or sepsis; Grade 4 local gangrene (toe, heel or forefoot dorsum), characterized by ischemic gangrene, usually accompanied by neuropathy; ④ Strong willingness to save limbs and good condition of popliteal artery of affected limbs; ⑤ There were no serious complications and other serious diseases, and there was no absolute surgical contraindication; ⑥ Patients were informed and voluntarily participated in this study.

排除标准:

①患肢腘动脉完全阻塞,无侧支循环建立;②静脉栓塞、淋巴管炎等动静脉血管性病变患者;③心、肺、肝、肾等脏器功能严重不全患者;④糖尿病急性并发症,如糖尿病高渗性昏迷、糖尿病酮症及酮症酸中毒、乳酸性酸中毒等;⑤合并恶性肿瘤者;⑥孕妇或哺乳期妇女。

Exclusion criteria:

① The popliteal artery of the affected limb was completely blocked and no collateral circulation was established; ② Patients with arteriovenous vascular diseases such as venous embolism and lymphangitis; ③ Patients with severe dysfunction of heart, lung, liver, kidney and other organs; ④ Acute complications of diabetes, such as hypertonic coma of diabetes, ketosis and ketoacidosis of diabetes, lactic acidosis, etc; ⑤ Patients with malignant tumor; ⑥ Pregnant or lactating women.
