研究目的：

通过提高患者智谋水平，降低MHD患者慢性病共存治疗负担，有效改善患者的预后及生活质量，为今后MHD患者慢性病共存治疗负担干预提供新的理论依据和方法借鉴。  

Objectives of Study：

In order to improve the level of resourcefulness, reduce the treatment burden for hemodialysis Patients with multimorbidity and provide new theoretical basis and methods for the intervention of coexisting treatment burden of hemodialysis patients in the future.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄18岁以上，接受血液透析≥3个月；②每周接受血液透析治疗≥2次；③具有独立的阅读和思考能力,愿意合作，能够进行正常的语言沟通；④意识清楚，知晓自己的诊断且同意参与本研究，签署知情同意书。⑤除了慢性肾脏病外，合并患13种慢性病（高血压病、血脂异常、糖尿病或血糖升高、癌症等恶性肿瘤[不包含轻度皮肤癌] 、慢性肺部疾患、肝脏疾病、心脏病、中风、肾脏疾病、胃部疾病或消化道系统疾病、情感及精神方面疾病、关节炎或风湿病以及哮喘）的1种或1种以上慢性病。⑥慢性病共存治疗负担得分≥10分。⑦智谋得分≤83分。

Inclusion criteria

1 Aged ≥18 years
2 Hemodialysis therapy was performed for at least 3 months, and at least 2 per week
3 Have independent reading and thinking ability, willing to cooperate, able to carry on normal language communication
4 Clear awareness, knowing their diagnosis, agreeing to participate in the study and signing informed consent
5 with one or more chronic morbidities from the specified list
6 The score of Chinese Multimorbidity Treatment Burden Questionnaire was ≥10
7 The score of Resourcefulness Scale ≤83

排除标准：

①病情不稳定、近期有器官移植或其他大手术；②同时进行腹膜透析的患者；③已行肾移植手术患者；④同时参与其他研究。

Exclusion criteria：

1 Unstable condition, have had or plan to have major surgery within the nearly 1 month
2 Peritoneal dialysis
3 renal transplantation were also performed
4 Also participate in other clinical trials
