研究目的：

目前对于病变侵及粘膜下层以下的进展期结肠癌患者，及部分术前通过白光内镜、NBI、超声内镜、CT等考虑粘膜内的早期结肠癌患者行内镜下粘膜剥离术（ESD），部分患者因分期不足，切源有病变残留，上诉患者目前采用的首选治疗均为外科手术，创伤大、手术相关并发症较多、增加患者痛苦。本研究基于NOTES相关技术行内镜下病变肠壁全层切除，术后采用改良的内镜下“8”字缝合进行肠管的端端吻合，进一步评估该手术方法的安全性、可行性。  

Objectives of Study：

Endoscopic mucosal dissection (ESD) is currently performed for patients with advanced colon cancer who have lesions infiltrating the submucosa and early colon cancer patients who have considered mucosa through white light endoscope, NBI, endoscopic ultrasound, CT, etc. Due to insufficient staging, some patients have residual lesions at the source. Surgical procedures are currently the first choice for appealing patients, with large trauma, more surgical-related complications, and increased patient suffering. This study was based on NOTES-related techniques to perform a full-thickness resection of the intestinal wall of the endoscope. Postoperative end-to-end anastomosis of the bowel was performed using a modified endoscopic "8" suture.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

术前通过内镜、病理、超声内镜、腹部增强CT等检查明确为左半结肠癌，术前分期为T1b深浸润-T2N0M0，术前与患者及家属充分沟通手术方式，术中、术后存在的风险及并发症、手术获益，可能中转外科手术可能，经同意签字后开展此手术。

Inclusion criteria

Preoperatively, he was confirmed to be left colon cancer by endoscopy, pathology, endoscopic ultrasound, abdominal enhanced CT, etc. The preoperative staging was T1b deep infiltration-T2N0M0, and the patient and his family were fully communicated with the surgical method before and after the operation. Existing risks and complications, surgical benefits, may be transferred to surgery may be carried out after consent to sign this operation.

排除标准：

1.存在严重心肺疾病，不适合内镜检查；
2.已出现严重的并发症;
3.术前排除糖尿病、凝血功能障碍等基础疾病。

Exclusion criteria：

1. There are serious cardiopulmonary diseases, not suitable for endoscopy;
2. Serious complications have occurred;
3. Pre-exclusion of basic diseases such as diabetes and coagulopathy.
