研究目的：

本研究通过双向凝胶电泳技术（2-DE）、基质辅助激光解析电离飞行时间串联质谱技术（MALDI-TOF-MS/MS）与非靶向代谢组学液相色谱-质谱技术，对急性心肌梗死患者的罪犯血栓斑块蛋白质组及血清代谢特征进行系统分析，筛选与急性心肌梗死的罪犯斑块分型相关的差异蛋白及代谢物，并与AMI患者临床特征改变相关联，筛选和验证灵敏度高、专属性强的AMI临床罪犯斑块分型血清代谢标志物，优化和构建准确率高、适用性广、方便快捷的诊断模型，为AMI的早期诊断鉴别、精准治疗提供科学依据。  

Objectives of Study：

In this study, two-dimensional gel electrophoresis, matrix-assisted laser desorption ionization time-of-flight tandem mass spectrometry, and non-targeted metabolomics liquid chromatography-mass spectrometry were used to analyze the proteomic and serum metabolic characteristics of culprit thrombus plaques in patients with acute myocardial infarction. Systematic analysis, screening of differential proteins and metabolites related to the culprit plaque typing of acute myocardial infarction, and correlation with changes in clinical characteristics of AMI patients, screening and validation of AMI clinical culprit plaque typing serum with high sensitivity and specificity Metabolic markers, optimizing and constructing a diagnostic model with high accuracy, wide applicability, convenience and rapidity, provide a scientific basis for the early diagnosis, identification and precise treatment of AMI.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄大于18岁的患者；
2. 明确诊断为急性心肌梗死，且需要进行介入治疗的患者；急性心肌梗死诊断标准：
（1）STEMI诊断标准：cTn>99%正常参考值上限（ULN）或CK-MB>99hULN，心电图表现为ST段弓背向上抬高，伴有下列情况之一或以上者：持续缺血性胸痛；超声心动图显示节段性室壁活动异常；冠状动脉造影异常；
（2）NSTEMI诊断标准：cTn>99%ULN或CK-MB>99%ULN，并同时伴有下列情况之一或以上者：持续缺血性胸痛；心电图表现为新发NSTEMI的ST段压低或T波低平、倒置；超声心动图显示节段性室壁活动异常；冠状动脉造影异常；
3. 确定罪犯斑块，在该研究中，对于罪犯病变的确定是由冠状动脉造影以及ECG检查结果共同确定的。

Inclusion criteria

1. Patients older than 18 years;
2. Patients who are clearly diagnosed with acute myocardial infarction and require interventional therapy; diagnostic criteria for acute myocardial infarction:
(1) STEMI diagnostic criteria: cTn>99% upper limit of normal reference value (ULN) or CK-MB>99hULN, ECG showed ST segment arched dorsal elevation, accompanied by one or more of the following conditions: persistent ischemic chest pain; ultrasound; Cardiogram showing segmental wall motion abnormalities; coronary angiography abnormalities;
(2) Diagnostic criteria for NSTEMI: cTn > 99% ULN or CK-MB > 99% ULN, accompanied by one or more of the following conditions: persistent ischemic chest pain; electrocardiogram showing ST segment depression or T wave low in new-onset NSTEMI Flat, inverted; echocardiogram showing segmental wall motion abnormalities; coronary angiography abnormalities;
3. Determine the culprit plaque, in this study, the determination of the culprit lesion was determined by coronary angiography and ECG examination results.

排除标准：

1. 明确诊断不稳定性心绞痛的患者（UA）,不稳定性心绞痛诊断标准：cTn阴性，缺血性胸痛，心电图表现为一过性ST段压低UA或T波低平、倒置，少见ST段抬高（血管痉挛性心绞痛）；
2. 心源性休克、终末期肾脏病、严重肝功能损害、对造影剂过敏以及对阿司匹林、氯吡格雷过敏者；
3. 慢性完全闭塞性病变、左主干病变、严重钙化病变、血管严重迂曲等进行OCT检查存在困难的病变；
4. 罪犯病变处已行冠状动脉旁路移植术治疗或既往接受过支架植入者。

Exclusion criteria：

1. Patients with a definite diagnosis of unstable angina pectoris (UA), the diagnostic criteria for unstable angina pectoris: cTn negative, ischemic chest pain, electrocardiogram showing transient ST segment depression UA or T wave flat and inverted, rare ST Segment elevation (vasospastic angina);
2. Cardiogenic shock, end-stage renal disease, severe liver damage, allergy to contrast agents, and allergy to aspirin and clopidogrel;
3. Chronic total occlusive disease, left main stem disease, severe calcified disease, severe tortuosity of blood vessels, etc., for which it is difficult to perform OCT examination;
4. Those who have received coronary artery bypass grafting or stent implantation in the culprit lesion.

本研究纳入来我院治疗的符合纳入标准的急性心肌梗死患者并签订知情同意书。根据罪犯病变特征，将患者分成斑块破裂组与斑块侵蚀组。因此，本研究不进行随机分组。

This study included patients with acute myocardial infarction who met the inclusion criteria and signed informed consent. According to the characteristics of the culprit lesions, the patients were divided into plaque rupture group and plaque erosion group. Therefore, randomization was not performed in this study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据及时、完整、正确、清晰地载入病例报告表。录入采用相应的数据库系统双人双机录入，之后对数据库进行两遍比对。电子数据文件分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。

Based on the original observation records, researchers will completely write accurate data into case report forms in time.Input the data into corresponding database system by two special researchers with two computers respectively. After that, researchers compare two database twice and electronic data will be conserved and backup.
