研究目的：

比较基于触发点理论与基于传统中医理论选取针刺部位治疗足底痛伴下腰背痛的疗效差异及超声下足底筋膜的变化差异。  

Objectives of Study：

To compare the effectiveness of acupuncture based on myofascial trigger points and TCM theory for treating plantar fasciitis, and change in the plantar fascia.

药物成份或治疗方案详述：

将受试者随机分为两组，分别为触发点针刺组和常规穴位针刺组，两组均进行针刺治疗，除针刺穴位不同外，其他操作均相同。分组治疗方案如下： 1.触发点针刺组： (1)针刺部位： ①足弓和足中部疼痛：腓肠肌（足弓）、趾长屈肌（足中部靠前、大脚趾）、拇收肌（足前部行走时），针刺部位选取疼痛对应肌肉的触发点； ②足跟疼痛：比目鱼肌（足跟、小腿后部、踝后部）、足底方肌（足跟刺痛）、拇展肌、胫骨后肌（跟腱、可扩散至小腿后部、足跟和整个足底），针刺部位选取疼痛对应肌肉的触发点； ③下腰背部疼痛：臀中肌、腰大肌/髂肌、脊柱深层肌、脊柱表层肌、腰方肌、臀大肌、腹直肌、比目鱼肌，针刺部位选取疼痛对应肌肉的触发点。 (2)操作：使用0.30*0.40cm华佗牌一次性针灸针，根据疼痛部位选取相应触发点，垂直刺入约0.2-0.35cm，留针30分钟，每隔10分钟每穴行均匀提插、捻转手法三次。 2.常用穴位针刺组： (1)取穴： ①足底疼痛：承山、太溪、昆仑； ②下腰背痛：下腰背疼痛部位夹脊穴 (2)操作：使用0.30×0.40cm华佗牌一次性针灸针，垂直刺入疼痛对应部位双侧华佗夹脊穴约0.2-0.35cm，每垂直刺入约0.2-0.35cm，留针30分钟，每隔10分钟每穴行均匀提插、捻转手法三次。 两组患者均每周治疗3次，治疗2周，共6针次，随访12周。 

Description for medicine or protocol of treatment in detail：

Patients will be randomly divided into two groups, the myofascial trigger points group, and the TCM acupoints group. patients in both groups will receive the same acupuncture treatment except for needle points. The treatment protocol for each group is as follows: 1. myofascial trigger point group: acupuncture on trigger points corresponding to related muscle: ① pain in the arch and midfoot: gastrocnemius (arch), flexor digitorum longus (front midfoot, big toe), hallucinus adductor (when walking on the front of the foot), select the trigger point of the muscle corresponding to the pain at the acupuncture site; ②Heel pain: soleus (heel, back of the calf, back of the ankle), quadratus plantar (heel tingling), abductor pollicis, posterior tibialis (Achilles tendon, can spread to the back of the calf, heel and the entire sole), the acupuncture site selects the trigger point of the muscle corresponding to the pain; ③ Lower back pain: gluteus medius, psoas major /iliac muscle, deep spinal muscle, superficial spinal muscle, quadratus lumborum, gluteus maximus, rectus abdominis, soleus muscle. 2. TCM acupoints group: acupuncture on acupoints based on traditional Chineses Medicine. If patients have plantar heel pain, Chenshan (BL57), Taixi (KI3), and Kunlun (BL60) will be chosen. If patients have low back pain, Jiaji (EX-B2) will be chosen. Operation: both groups will vertically insert Huatuo disposable needles in corresponding points about 0.2-0.35cm (according to patients' figure), retain the needles for 30 minutes, and evenly lift and rotate each needle three times every 10 minutes. Patients in both groups will receive 6 acupuncture sessions in two weeks, then follow up for 12 weeks. 

纳入标准：

1．诊断为足底筋膜炎且足底不疼痛评分≥3分的患者；
2．伴有腰痛，腰痛程度不限；
3．18-60岁；
4．可遵循研究方案、签署知情同意书并自愿参与试验。

Inclusion criteria

1. Patients have plantar fasciitis and plantar heel pain over 3 points on a 0-10 numeric pain scale;
2. Patients accompanied with low back pain without limitation of pain intensity;
3. Age 18 to 60 years old;
4. Able to follow the study protocol, sign the informed consent, and voluntarily participate in the trial.

排除标准：

1．足部畸形（如扁平足、马蹄内翻足等）；
2．足部X线侧位片示巨大根骨骨赘形成；
3．腰部畸形（如严重腰椎间盘突出、腰椎滑脱、腰椎侧弯等）；
4．腰痛伴有下肢放射性疼痛；
5．脊柱或足部肿瘤、急性骨折或骨折病史；
6．内分泌疾病或自身免疫病（如1型或2型糖尿病、类风湿关节炎、通风、严重骨质疏松等；
7．腰骶部神经根病变、踝关节周围神经根病变；
8．需针刺部位存在皮肤感染；
9．血液系统疾病或重大系统性疾病。

Exclusion criteria：

1. Foot deformities (e.g., flat feet, clubfoot, etc.);
2. Lateral X-ray of the foot shows the formation of giant root osteophytes;
3. Lumbar deformity (e.g., severe lumbar disc herniation, lumbar spondylolisthesis, lumbar scoliosis, etc.);
4. Low back pain accompanied by lower extremity radiating pain;
5. Spinal or foot tumors, acute fractures, or a history of fractures;
6. Endocrine disease or autoimmune disease (such as type 1 or 2 diabetes, rheumatoid arthritis, ventilation, severe osteoporosis, etc.;
7. Lumbosacral radiculopathy, peripheral radiculopathy of ankle joint;
8. Skin infection at the site where acupuncture is required;
9. Blood system disease or major systemic disease

随机方案由不参与试验操作和评价的研究者负责编制，采用SPSS20.0软件生成随机数字，将随机数字分为两组，一组为腰部穴位组，另一组为肌筋膜触发点组，每组各31例。
