研究目的：

基于北京市海淀区和朝阳区正在推行的社区康复服务规范和国内外干预研究现状，本研究的总目标是通过社区康复干预改善精神分裂症患者的功能，减轻家庭照料负担，并验证社区康复干预是否具有成本效果，探讨其在中国推广的可行性。 具体来说包括两个研究问题： （1）社区康复服务干预是否可以在短期内提高精神分裂症患者治疗依从性和降低入院次数？在长期可以缓解患者疾病症状，改善功能与生命质量以及降低家庭照料负担？为回答这两个问题，本研究将利用影响评价（Impact evaluation，详见研究方法）这一因果推断方法来识别社区康复服务干预对患者及其家庭影响的效应。 （2）如果有影响，精神分裂症患者社区康复干预是否具有成本效果？是否值得推广？为回答这两个问题，本研究将进行干预方案的成本效果与成本效应，并结合我国精神分裂症人群的特点，分析推广干预方案的可行性。  

Objectives of Study：

The overall goal of the project is to improve quality of care for people with schizophrenia through community-based rehabilitation (CR) intervention. Ultimately, we hope that CR will be cost effective and can be scaled up in China. Objectives of the main study ? To evaluate if the CR intervention will improve adherence to antipsychotic treatment and reduce the hospital readmission rate in persons for schizophrenia (intermediate quality of care outcomes). ? To examine if the CR intervention will reduce symptoms of schizophrenia and improve physical function and quality of life for persons with schizophrenia (long-term quality of care outcomes). ? To analyze the cost-effectiveness of the CR intervention.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）18-60 岁；2）精神科医生根据ICD-10标准初步诊断为精神分裂症；3）患病时间不少于12个月；4）根据《严重精神障碍管理治疗工作规范》判定为病情稳定的居家患者；5）预期未来4年内居住在研究区域内。

Inclusion criteria

The inclusion criteria for participating in the trial are:
a) Trial participants must be between the ages of 18-60 years;
b) Have a primary diagnosis of schizophrenia by psychiatrists according to ICD-10 criteria;
c) Have had an illness duration of at least 12 months; and
d) Be residing within the study catchment area for the next 12 months.

排除标准：

排除患有严重躯体疾病、精神发育迟滞、共病双相情感障碍和重性抑郁障碍的受试者。

Exclusion criteria：

Ever been diagnosed with drug or alcohol dependence, organic brain disease, learning disability or hearing impairment.

本研究将按照以下步骤实施： 1）在海淀区和朝阳区各选择5个街道（镇），由社区卫生中心提供社区卫生服务； 2）每个街道（镇）随机抽取10个社区（村）， 得到100个社区（村）（2个区*5个街道（镇）*10个社区（村））的样本框； 3）从当地社区的信息系统中获取精神分裂症患者的社会人口特征、健康状况和患病信息； 4）选择两个社区（村）， 按照人口数量、年龄、收入、健康状况和精神分裂症患病率进行匹配； 5）确定匹配的两个社区（村）后，将其一随机分配为干预组社区（村）；另一组随机分配为对照组社区（村），得到50个干预组社区（村）和50个对照组社区（村）； 6）每个社区（村）约有12-15名精神分裂症患者，假设每个社区（村）的招募率为50%，我们从每个社区（村）各招募6名居家患者，共招募600名精神分裂症患者（干预组300名，对照组300名）。
