研究目的：

本研究拟开展单中心随机对照试验研究，拟收集Ⅲ型食管闭锁患儿40例，通过中心分层动态随机分配方案将患者随机分配至单纯结扎及缝扎组中，通过一系列主要和次要观察指标评估患儿术后并发症的发生情况，探讨更好的手术方式。  

Objectives of Study：

In this study, a single-center randomized controlled trial was planned to collect 40 children with type Ⅲ esophageal atresia, and randomly assign them to the group of simple ligation and suture through the central stratified dynamic random assignment scheme. A series of primary and secondary observation indicators were used to evaluate the occurrence of postoperative complications in these children, and to explore better surgical methods.

药物成份或治疗方案详述：

将符合纳入标准的患者通过中心分层动态随机分配方案将患者随机分配至缝扎及单纯结扎组中： A 组：胸腔镜手术下III型食管闭锁 远端气管食管瘘采用单纯结扎后离断的方式处理，余采用标准化治疗方案； B 组：胸腔镜手术下 III型食管闭锁远端气管食管瘘采用缝合结扎后离断的方式处理，余采用标准化治疗方案。 

Description for medicine or protocol of treatment in detail：

Patients meeting the inclusion criteria were randomly assigned to the suture and ligation alone group through the central stratified dynamic random assignment scheme: Group A: The distal tracheoesophageal fistula of type III esophageal atresia under thoracoscopic surgery was treated by simple ligation and dissociation, and the rest was treated by standard treatment. Group B: The distal tracheoesophageal fistula of type III esophageal atresia under thoracoscopic surgery was treated by suture and ligation and then severed, and the rest was treated by standard treatment. 

纳入标准：

1.2021年11月至2023年11月期间在首都医科大学附属北京儿童医院就诊，经临床评估和影像学检查初步诊断为Ⅲ型食管闭锁；
2.入院年龄≤7 天，出生体重≥2kg 并且≤4kg；
3.家属签字同意加入本研究；
4.签署知情同意。

Inclusion criteria

1. Received medical treatment in Beijing Children's Hospital Affiliated to Capital Medical University from November 2021 to November 2023, and was preliminarily diagnosed as type III esophageal atresia after clinical evaluation and imaging examination;
2. Age of admission <=7 days, birth weight 2-4kg;
3. The family members signed and agreed to join the study;
4. Sign informed consent.

排除标准：

1.合并重症肺炎，术前需要有创呼吸支持；
2.合并严重心脏畸形；
3.合并喉裂；
4.合并其他危及生命的重大畸形的；
5.纤维支气管镜检查发现食管近端与气管连接形成气管食管瘘；
6.不同意胸腔镜手术；
7.对治疗或临床研究依从性差者；
8.拒绝参加本研究者。

Exclusion criteria：

1. Complicated with severe pneumonia, invasive respiratory support was required before surgery;
2. Complicated with severe heart malformations;
3. Laryngeal fissure;
4. With other life-threatening major malformations;
5. Fiberoptic bronchoscopy found that the proximal esophagus was connected to the trachea and formed a tracheoesophageal fistula;
6. Do not agree with thoracoscopic surgery;
7. Poor compliance with treatment or clinical research;
8. Refused to participate in the study.
