分析天津市居民的常见病、慢性病的流性特征及变化趋势；探寻疾病的危险因素；构建个性化的疾病风险评估模型；评价卫生政策的实施效果；完善社区慢病管理的应对策略。  

Objectives of Study：

To analyze the flow characteristics and changing trends of common diseases and chronic diseases of Tianjin residents; to explore the risk factors of diseases; to build a personalized disease risk assessment model; to evaluate the implementation effect of health policies; to improve the coping strategies for community chronic disease management.

纳入标准：

1.年龄：大于18岁，男性或女性；
2.自愿参加并签署知情同意书。

Inclusion criteria

1. Aged over 18 years old men or women;
2. Voluntarily participating in our study and signing of informed consent.

排除标准：

1.项目地区流动人口或暂住居民；
2.驻扎在项目地区的部队及所属机关的工作人员（包括离退休者）；
3.患有严重精神疾病，无法正常交流人员；
4.正参加其它临床试验的患者；
5.研究人员认为其他原因不适合该项研究者。

Exclusion criteria：

1. Floating population or temporary residents in the project area;
2. The troops stationed in the project area and the staff of their affiliated agencies (including retirees);
3. Suffering from severe mental illness, unable to communicate with people normally;
4. Patients who are participating in other clinical trials;
5. The researcher believes that other reasons are not suitable for the researcher.
