研究目的：

联合应用Li-ESWT与脐带来源干细胞治疗方案改善PDE5i治疗效果欠佳的糖尿病ED患者勃起功能，并对其的有效性和安全性进行评估；  

Objectives of Study：

Combined application of Li-ESWT and umbilical cord derived stem cell therapy to improve erectile function in diabetic patients with poor PDE5i treatment, and to evaluate its efficacy and safety.

药物成份或治疗方案详述：

低能量体外冲击波（Low intensity extracorporeal shock wave, Li-ESW）具有一定的促进组织新生、血管生成、改善器官血液供应的治疗效果。Li-ESW可显著修复糖尿病ED中平滑肌细胞、血管内皮细胞、神经纤维等结构的病理损伤。间充质干细胞（Mesenchymal stem cells, MSCs）在糖尿病ED动物模型和临床试验中均显示出良好的安全性和有效性。MSCs发挥治疗作用基于其较强的旁分泌能力，抑制血管内皮细胞凋亡，促进血管内皮细胞功能恢复。 

Description for medicine or protocol of treatment in detail：

Low intensity extracorporeal shock wave (Li ESW) has a certain therapeutic effect of promoting tissue regeneration, angiogenesis and improving organ blood supply. Li-ESW can significantly repair the pathological damage of smooth muscle cells, vascular endothelial cells and nerve fibers in diabetic ED. Mesenchymal stem cells (MSCs) has shown good safety and efficacy in diabetic ED animal models and clinical trials. MSCs play a therapeutic role. Based on its strong paracrine ability, MSCs inhibit the apoptosis of vascular endothelial cells and promote the functional recovery of vascular endothelial cells. 

纳入标准：

①年龄为20到60岁经临床确诊为勃起功能障碍患者；
②PDE5i治疗效果欠佳的糖尿病ED患者；
③患者主观需要进行治疗，并能够配合随访和治疗的患者；
④无严重慢性病

Inclusion criteria

①Patients aged 20 to 60 years and clinically diagnosed with erectile dysfunction;

②Diabetic ED patients with poor therapeutic effect to PED5i;

③Patients who subjectively need treatment and can cooperate with follow-up and treatment;

④No serious chronic disease

排除标准：

①年龄<20岁或>60岁；
②非糖尿病ED；
③存在急、慢性病史、重大外伤及手术史等；
④存在不稳定心肌梗死及脑梗病史；
⑤存在心源性休克、重度心力衰竭、严重室性心律失常等病史；
⑥存在严重疾病如恶性肿瘤、慢性肝肾疾病等。

Exclusion criteria：

①Age < 20 years or > 60 years;
②Non diabetic ED;;
③Patients with acute and chronic medical history, major trauma and operation history;
④Patients with history of unstable myocardial infarction and cerebral infarction;
⑤Patients with history of cardiogenic shock, severe heart failure and severe ventricular arrhythmia;
⑥Patients with serious diseases such as malignant tumors and chronic liver and kidney diseases.
