研究目的：

本课题拟基于糖尿病肾病（DKD）大样本生物样本库，应用RNA 高通量检测技术，探讨DKD 尿液脱落细胞转录组学特征，筛选并验证早期诊断及预测DKD 进展的生物标志物。运用机器学习算法，融入当前以微量白蛋白尿测定为基石的DKD 早期诊断体系，构建DKD 精准诊断和预后分层的多指标分类器。开发联合指标检测试剂盒，经过系统评价及优化，临床应用并推广。  

Objectives of Study：

This project intends to explore the transcriptome characteristics of DKD urine exfoliated cells and to screen and verify the biomarkers for early diagnosis and prediction of DKD progress, based on the large cohort biobank and RNA high-throughput detection technology. We plan to use machine learning algorithm to integrate into the current DKD early diagnosis system of microalbuminuria measurement to construct a multi-index classifier with accurate diagnosis and hierarchical prognosis of DKD. In the end, to development of the combined indicator detection kit for clinical application and promotion after systematic evaluation and optimization.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 年龄18岁~75岁；
(2) 符合2型糖尿病诊断；
(3) 糖尿病视网膜病变伴有微量白蛋白尿(ACR 30~300 mg/g，UAE 30~300mg/24h)；
或ACR> 300mg/g，UAE> 300mg/24h，伴有或不伴有视网膜病变；
或eGFR<60ml/min，伴有或不伴有视网膜病变, 排除原发性肾小球疾病和其他继发性肾脏病。
(4) 临床资料和实验室资料完整。

Inclusion criteria

(1) Aged 18 ~75 years;
(2) Meet the diagnosis of type 2 diabetes;
(3) Diabetic retinopathy with microalbuminuria (ACR 30~300mg/ g, UAE 30~300mg/24h);
Or ACR> 300mg/g, UAE> 300mg/24h, with or without retinopathy;
Or eGFR<60ml/min with or without retinopathy, excluding primary glomerular disease and other secondary renal diseases;
(4) Complete clinical and laboratory data.

排除标准：

(1) 合并其他肾脏疾病；
(2) 正在接受血液净化治疗；
(3) 正在接受糖皮质激素或其他免疫抑制剂治疗；
(4) 合并尿路感染等干扰尿沉渣检查结果的疾病。

Exclusion criteria：

(1) Combined with other kidney diseases;
(2) Receiving blood purification treatment;
(3) Being treated with glucocorticoid or other immunosuppressive agents;
(4) Combined with urinary tract infection or other diseases that interfere with the results of urine sediment examination.

1. 横断面研究 横断面研究采用分层随机方法。由研究者冯松涛按照每3个月时间间隔将研究对象进行分层，然后在每一层内进行单纯随机抽样。在每一层的N个研究对象的总体中，利用随机数字法按照相同比例抽取n个研究对象组成样本。 2. 队列研究 将DKD作为研究组，糖尿病不合并肾病者作为对照组，两组队列均进行分层抽样。由研究者冯松涛按照每三个月的时间间隔将进行随访的患者进行分层，然后在研究组和对照组进行单纯随机抽样。在每一层的N个研究对象总体中，利用随机数字法按照相同比例抽取n个研究对象，组成研究队列。

DKD was used as the study group and diabetic patients without nephropathy as the control group. Stratified sampling was conducted in both groups.The follow-up patients were stratified by researcher Feng songtao at intervals of three months and then randomly sampled in both groups by random number method.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1. 记录研究对象的基线资料，包括个人信息，临床特征和实验室检查结果等，及时记录本项目进行的相关尿液、血液标志物检测结果以及随访时监测指标的结果。 2. 将研究对象的基线临床资料，相关尿液、血液标志物检测结果，以及患者的临床随访资料等进行电子表格输入和保存，以进行后续的统计分析和临床预测模型建模。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Record the baseline data of the study subjects, including personal information, clinical characteristics and laboratory examination results. Record the results of urine and blood markers test and monitoring indicators of the study subjects during follow-up. 2. The baseline clinical data of the study subjects, the results of relevant urine and blood markers detection, and the clinical follow-up data of the patients were input and saved in the electronic form for subsequent statistical analysis and clinical prediction model modeling.
