研究目的：

1、基于既往成果、文献分析、专家问卷等，提出脊柱病 “椎骨错缝”影像学评价要素的基本架构，遵循规范流程而编制临床运用简便、有效的评价量表。 2、基于临床病例接受脊柱手法治疗后的改善效果，分析量化表格评分对判断“椎骨错缝”及其纠正程度的敏感性、稳定性。  

Objectives of Study：

1. Based on previous results, literature analysis, expert questionnaires, etc., the basic framework of the imaging evaluation elements of spinal disease "vertebrae dislocation" is proposed, and a simple and effective evaluation scale for clinical use is prepared following a standardized process. 2. Based on the improvement effect of clinical cases after receiving spinal manipulation treatment, the sensitivity and stability of quantitative table score in judging "vertebral dislocation" .

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合腰椎间盘突出症诊断标准；2.CT或MRI显示单纯一个病变节段：L4-5突出；3.年龄35-55岁的中年人；4.发作时程≥2周的非急性期；5.触诊见：患椎棘突向左或右偏歪、患椎上下棘间隙宽窄不等、患椎棘突旁压痛或伴有下肢放射痛、患椎棘上韧带触之钝厚；6.查体见：下腰椎弧度、骶骨倾斜度、骨盆水平等出现腰骶部力学失衡改变；7.治疗前后均完成了站立位腰椎正侧位Ｘ线片的检查、以及仰卧位MR检查。

Inclusion criteria

1. Meeting the diagnostic criteria for lumbar disc herniation;
2. CT or MRI showed only one lesion segment: L4-5 protruded; 3. Middle aged people aged 40-55;
4. Non acute phase with onset duration ≥ 2 weeks; 5. Palpation findings: the spinous process of the affected vertebrae deviates to the left or right, the space between the upper and lower spines of the affected vertebrae is not equal in width, the tenderness beside the spinous process of the affected vertebrae is accompanied by radiation pain of the lower limbs, and the contact of the supraspinous ligament of the affected vertebrae is blunt and thick; 6. The physical examination showed that the lower lumbar curvature, sacral inclination, pelvic level, and other changes in lumbosacral mechanical imbalance; 7. X-ray examination of lumbar spine in standing position and MR examination in supine position were completed before and after treatment.

排除标准：

1.腰椎间盘突出症与腰椎管狭窄症合并发病的患者；2.多个节段腰椎间盘突出症的患者；3.发作2周内神经根性疼痛明显、或行走困难的急性期患者；4.足下垂等腓总神经麻痹，或尿便障碍、鞍区麻木等马尾神经损伤的患者；5.合并椎体滑脱、强直性脊柱炎、代谢性骨病、骨质疏松、肿瘤、结核、骨折、感染等影响脊柱形态的患者。

Exclusion criteria：

1. Patients with lumbar disc herniation and lumbar spinal stenosis; 2. Patients with multiple segmental lumbar disc herniation; 3. Acute stage patients with obvious nerve root pain or difficulty in walking within 2 weeks after onset; 4. Patients with common peroneal nerve paralysis such as foot drop, or cauda equina nerve injury such as urinary and fecal disorders and numbness in sella region; 5. Patients with spondylolisthesis, ankylosing spondylitis, metabolic bone disease, osteoporosis, tumor, tuberculosis, fracture, infection and other factors that affect the shape of the spine.
