研究目的：

观察胺碘酮对比琥珀酸美托洛尔对心衰合并持续性快速房颤患者的疗效。  

Objectives of Study：

To observe the effect of amiodarone and metoprolol succinate on patients with heart failure complicated with persistent rapid atrial fibrillation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄55~75岁；
2. 入院前所有患者既往均规律服用金三角药物琥珀酸美托洛尔+培哚普利+螺内酯，并且达到最大耐受剂量；
3. 入院时心电图提示：快速房颤（心率＞100bpm）且房颤持续时间超过7天，达到干体重后动态心电图平均心室率＞100次/分；
4. 入院时纽约心功能分级达到IV级需暂时停用琥珀酸美托洛尔；
5. 左心室射血分数（LVEF）值＜40%，proBNP＞1200pg/ml；
6. 所有入组患者均行食道超声证实心房无血栓形成；
7. 近1年内无房颤消融治疗计划。

Inclusion criteria

1. Age 55 to 75 years old;
2. Before admission, all patients used to take the Golden Triangle drug metoprolol succinate + perindopril + spironolactone regularly, and reached the maximum tolerated dose;
3. ECG indication at admission: rapid atrial fibrillation (heart rate > 100 BPM) and the duration of atrial fibrillation exceeds 7 days. After reaching dry weight, the average ventricular rate of ambulatory ECG is > 100 beats/min;
4. Metoprolol succinate should be temporarily stopped when the cardiac function grade of New York City reaches grade IV at admission;
5. Left ventricular ejection fraction (LVEF) < 40%, proBNP > 1200pg/ml;
6. All enrolled patients underwent esophageal ultrasound to confirm that there was no atrial thrombosis;
7. There was no ablation treatment plan for atrial fibrillation in recent 1 year.

排除标准：

1. 需要外科干预的瓣膜性心脏病；
2. 合并甲亢或经确诊的甲亢性心脏病；
3. 存在使用口服胺碘酮片的禁忌症；
4. 严重的肝肾功能不全；
5. 严重的室内传导阻滞及完全性左束支传导阻滞。

Exclusion criteria：

1. Valvular heart disease requiring surgical intervention;
2. Complicated with hyperthyroidism or diagnosed hyperthyroid heart disease;
3. There are contraindications to the use of oral amiodarone tablets;
4. Severe hepatic and renal insufficiency;
5. Severe indoor conduction block and complete left bundle branch block.
