研究目的：

阻生下颌第三磨牙拔除导致邻近第二磨牙牙周组织破坏是临床治疗面临的一大难题。第三磨牙拔除术中切口设计是第一步，且有研究表明，不同切口设计对牙周组织的影响有差异。 为了尽量减少阻生下颌第三磨牙拔除对邻近第二磨牙牙周组织的影响，本研究通过设计不同的切口，比较在第三磨牙拔除后邻牙牙周组织及龈下菌群的变化，为第三磨牙拔除提供新的思路。  

Objectives of Study：

The extraction of impacted mandibular third molars leads to the destruction of the periodontal tissue adjacent to the second molars, which is a major problem in clinical treatment. Incision design is the first step in third molar extraction, and studies have shown that different incision designs have different effects on periodontal tissue. In order to minimize the impact of the extraction of the impacted mandibular third molar on the periodontal tissue of the adjacent second molar, this study designed different incisions to compare the changes in the periodontal tissue and subgingival flora of the adjacent teeth after the extraction of the third molar. Third molar extraction provides new ideas.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）患者全身健康，所有拔除的阻生下颌第三磨牙均为完全粘膜下阻生，阻生下颌第三磨牙和相邻第二磨牙局部无炎症，无盲袋；（2）根据全景X线片，选择技术难度相似的BII类近中或水平IMTMs患者；（3）阻生下颌第三磨牙牙冠近中最凸点至相邻第二磨牙远中颈部最凹点的距离（即智齿前间隙）为0mm，相邻第二磨牙远中牙槽嵴距牙釉质-牙槽骨交界处4-5mm；(4)阻生下颌第三磨牙及相邻第二磨牙无任何局部炎症或其他病变；（5）第二磨牙位于牙弓内，呈垂直位，第三磨牙位于第二磨牙远中，阻生下颌第三磨牙颌平面与下颌平面的夹角为11°～100°。

Inclusion criteria

（1) The patient's whole body is healthy, all the extracted impacted mandibular third molars are completely submucosal impacted, and the impacted mandibular third molar and adjacent second molar have no local inflammation and no blind pocket; (2) According to the panoramic X-ray (3) The distance from the most convex point of the crown of the impacted mandibular third molar to the most concave point of the distal neck of the adjacent second molar (i.e. the wisdom tooth) Anterior space) is 0mm, and the distal alveolar ridge of the adjacent second molar is 4-5mm away from the enamel-alveolar bone junction; (3) The impacted mandibular third molar and adjacent second molar do not have any local inflammation or other Lesion; (4) The second molar is located in the dental arch and is vertical, the third molar is located in the distal part of the second molar, and the angle between the jaw plane and the mandibular plane of the impacted mandibular third molar is 11°-100°.

排除标准：

①术中清创及手术时间>45min；②术后使用了抗生素及漱口水；③伴有颌骨骨折、慢性全身性疾病的患者；④吸烟的习惯；⑤怀孕；⑥慢性牙周病患者；⑦术前90天内服用类固醇或非类固醇抗炎药、益生菌、抗生素和双磷酸盐的患者；⑧颌骨囊肿或肿瘤患者。

Exclusion criteria：

①Intraoperative debridement and operative time >45min; ②Those on post-operative antibiotics and mouthwash; ③patients with concomitant jaw fractures/ chronic systemic illness; ④the habit of smoking ⑤pregnancy; ⑥patients with chronic periodontal disease; ⑦patients on steroids or non-steroidal anti-inflammatory drugs, probiotics, antibiotics and bisphosphonates within the past 90 days preoperatively; ⑧patients with a cyst or tumor of the jaw.
