研究目的：

评价上海以心医疗器械有限公司MitraFix? 经导管人工二尖瓣系统治疗传统外科手术高危的重度二尖瓣反流的有效性及安全性。  

Objectives of Study：

Evaluation of efficacy and safety of MitraFix? transcatheter artificial mitral valve system of MitrAssist Lifesciences Limited in the treatment of high-risk severe mitral regurgitation in traditional surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1)二尖瓣反流≥3+；
2)年龄≥18周岁；
3)传统外科手术高危的患者：STS二尖瓣置换死亡风险≥8%；或存在2个或以上虚弱指数指标；或存在2个或以上术后仍不能改善的主要脏器功能不全；或合并有不适合外科手术的其他疾病或因素；
4)研究者评估解剖适合经心尖植入MitraFix? 经导管人工二尖瓣系统；
5)能够理解试验的目的，自愿参加并签署知情同意书，愿意接受相关检查和临床随访的患者。

Inclusion criteria

1) Mitral regurgitation≥3+;
2) Age ≥ 18 years old;
3) Patients at high risk of traditional surgery: death risk of STS mitral valve replacement ≥ 8%; Or there are 2 or more frailty indexes; Or there are two or more major organ dysfunction that can not be improved after operation; Or combined with other diseases or factors unsuitable for surgery;
4) The investigator assessed anatomical suitability for transapical implantation of MitraFix? transcatheter artificial mitral valve system;
5) Patients who can understand the purpose of the trial, voluntarily participate in and sign informed consent, and are willing to accept relevant examination and clinical follow-up.

排除标准：

1) 二尖瓣重度狭窄；
2) 二尖瓣瓣叶和/或瓣环严重钙化；
3) 存在心脏二尖瓣植入物；
4) 感染性心内膜炎或者提示存在活动感染的患者；
5) 合并严重的未有效治疗的冠脉病变或90天内发生过急性心肌梗死；
6) 严重肺动脉高压患者（超声估测肺动脉收缩压 >70mmHg）；
7) 三尖瓣环收缩期位移（TAPSE)＜12；
8) 合并需要外科或介入治疗 的三尖瓣、主动脉瓣或严重大血管病变；
9) 左心射血分数<30%；
10) 患者极度虚弱不能耐受经心尖路径手术或处于休克状态需要循环支持；
11) 30天内植入冠脉支架、CRT、CRT-D或植入式的转复除颤器；
12) 接受慢性透析的患者；
13) 明确的凝血功能障碍及严重凝血系统疾病的患者；
14) 有明确抗凝药物使用禁忌的患者；
15) 超声心动图发现任何心内团块、左心室或心房血栓；
16) 进行经食道超声检查存在禁忌；
17) 已知对造影剂，镍钛记忆合金或牛源性产品过敏；
18) 患者预期寿命<12个月；
19) 研究者判断不适合建立心尖入路；
20) 筛选前 3个月内曾参加其他器械或药物临床研究。

Exclusion criteria：

1) Severe mitral stenosis;
2) Severe calcification of mitral leaflet and /or annulus;
3) Presence of cardiac mitral implants
4) Patients with infective endocarditis or suggesting active infection;
5) Complicated with serious untreated coronary artery disease or acute myocardial infarction within 90 days;
6) Patients with severe pulmonary hypertension (pulmonary systolic pressure estimated by ultrasound > 70mmHg)
7) Tricuspid annular systolic displacement (TAPSE) < 12;
8) Severe aortic or tricuspid valve disease requiring surgical intervention;
9) Left ventricular ejection fraction < 30%;
10) The patient is extremely weak and cannot tolerate the transapical approach or is in shock and needs circulatory support;
11) Implantation of coronary stent, CRT, CRT-D or implantable cardioverter defibrillator within 30 days;
12) Patients receiving chronic dialysis
13) Patients with definite coagulation dysfunction and severe coagulation system diseases;
14) Patients with clear contraindications to the use of anticoagulant drugs;
15) Echocardiography revealed any intracardiac mass, left ventricular or atrial thrombus;
16) There are contraindications in transesophageal ultrasonography;
17) Known allergy to contrast agents, nitinol or bovine products;
18) Life expectancy<12 months;
19) The investigators judged that it was not suitable to establish the transapical approach;
20) Participated in clinical research of other devices or drugs within 3 months before screening.
