研究目的:

本研究旨在探讨AIS患者脊柱影像学指标与肺功能、心肺运动功能的相关性,分析AIS患者心肺运动功能降低情况,为有效的提升AIS患者心肺运动功能提供理论依据。  

Objectives of Study:

This study aims to explore the correlation between spinal imaging indexes and pulmonary function and cardiopulmonary exercise function in adolescent idiopathic scoliosis patients, analyze the decline of cardiopulmonary exercise function in adolescent idiopathic scoliosis patients, and provide a theoretical basis for effectively improving the cardiopulmonary exercise function in adolescent idiopathic scoliosis patients.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

(1)年龄在10-30岁。(2)明确诊断为AIS的患者:使用Cobb角法测量站立位全脊柱冠状面X线片,Cobb角大于10°即为脊柱侧凸(3)Cobb角在10°-45°的单胸弯(顶椎在T10以上)、胸腰双弯(主胸弯顶椎在T10以上)或单腰弯的患者。

Inclusion criteria

(1) Aged 10-30. (2) Patients with definite diagnosis of AIS: coronal radiographs of the whole spine in standing position were measured by the Cobb Angle method, and scoliosis was defined as Cobb Angle greater than 10°. (3) Patients with single thoracic curve (apical vertebrae above T10), double thoracolumbar curve (apical vertebrae above T10 of the main thoracic curve) or single lumbar curve with Cobb Angle between 10° and 45°.

排除标准:

(1)先天性、姿势性、神经肌肉性或者其他疾病(如神经纤维瘤病、马凡综合症、骨发育不良、代谢性或者内分泌疾病等)原因引起的脊柱侧凸。(2)首次就诊前有康复治疗史或者手术史。(3)既往有其它呼吸系统、骨骼肌肉或者神经系统疾病。(4)既往有先天性心脏病、心律失常等可影响心肺运动功能的疾病。

Exclusion criteria:

(1) Scoliosis caused by congenital, postural, neuromuscular or other diseases (such as neurofibromatosis, Marfan syndrome, bone dysplasia, metabolic or endocrine diseases). (2) The patient had a history of rehabilitation or surgery before the first visit. (3) Previous respiratory, musculoskeletal or neurological diseases. (4) Previous congenital heart disease, arrhythmia and other diseases that can affect the cardiopulmonary function.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

采用EpiData3.1软件进行数据的录入,首先查找异常值,核对及排除数据录入的错误,对离群值进行删除。采用K-S检验验证变量是否符合正态分布,同时进行方差齐性Levene检验,正态分布的数据描述用平均值±标准差(Mean±SD),不符合正态分布的变量需进行对数的转换以符合正态分布的假设。不同性别患者基本信息的比较和不同侧凸方向患者肺功能和CPET指标的比较采用独立性t检验,将脊柱侧凸的影像学指标与肺功能和CPET指标进行Pearson相关分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

EpiData3.1 software was used for data entry. First, outliers were searched, errors in data entry were checked and excluded, and outliers were deleted. The K-S test was used to verify whether the variables were in accordance with the normal distribution, and the Levene test for homogeneity of variance was also performed. The data with normal distribution were described by Mean± standard deviation (Mean±SD), and the variables without normal distribution were log-transformed to meet the assumption of normal distribution. The basic information of patients with different genders and the pulmonary function and CPET indexes of patients with different scoliosis orientations were compared by independent t-test. Pearson correlation analysis was performed between the imaging indexes of scoliosis and the pulmonary function and CPET indexes.
