研究目的：

分析益生菌干预是否能够降低ICU患者接受肠内营养治疗不耐受的发生率，并从患者肠道微生态水平解析其潜在的机制。  

Objectives of Study：

To analyze whether probiotic intervention can reduce the incidence of intolerance to enteral nutrition therapy in ICU patients, and to analyze the underlying mechanism from the intestinal microecology level of patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄≥18周岁。
2. 需要肠内营养至少一周以上的患者。
3. 能够按照研究人员的要求提供粪便样本的患者。

Inclusion criteria

1. Be at least 18 years old.
2. Patients requiring enteral nutrition for at least one week.
3. Patients who are able to provide stool samples as requested by the researchers.

排除标准：

1. 纳入研究前1个月内有益生菌使用史
2. 纳入研究前患有严重消化道类疾病
3. 预后过差或病情严重，经临床医生判断不适合参加本研究
4. 对肠内营养方案中物质过敏的患者

Exclusion criteria：

1. The use history of beneficial bacteria in 1 month before the study was included
2. Had severe gastrointestinal diseases before inclusion in the study
3. Patients with poor prognosis or serious condition are not suitable for this study as judged by clinicians
4. Patients who are allergic to substances in enteral nutrition regimens

随机化方法：由于本研究中并未限制患者收治ICU的原因，导致疾病原因数量较多较为复杂，因此本研究使用区组随机化分组，收治ICU的患者的入院原因通常可以归类为以下十种疾病之一：1.消化道疾病； 2.肝脏疾病；3. 胆囊、胆道或胰腺疾病 4. 呼吸道疾病；5. 心血管疾病；6.肾脏疾病；7.神经性疾病；8.恶心肿瘤；9.血液学或免疫学疾病；10.代谢性疾病。由于预计总入组240名患者，因此每个区组的长度选为24，满足区组长度一般为组数的2倍以上的条件。实验组和对照组的所有可能排列为2704156种，由于分组数量过于庞大，使用随机化软件或程序（Python等）给每种可能排列的区组分配抽样编号，并直接抽取随机数列，获得特定区组分配的随机序号，找到对应的分配方案。

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究为电子化数据收集研究，患者CRF将在研究过程中被研究人员直接录入数据管理系统的数据库中。受试者的样本经检验科或第三方检验机构分析后，分析结果直接提交给数据管理员，并批量录入数据管理系统。在数据清理的过程中，异常或缺失的数据将由数据管理员向研究人员提出疑问并得到最终解决。本研究的数据由本项目组和合作单位共同保存并相互监督，以防止数据丢失、修改或不当使用。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

This study is an electronic data collection study, and patients' CRF will be directly entered into the database of the data management system during the study. After the samples of the subjects are analyzed by the inspection department or third-party inspection institutions, the analysis results will be directly submitted to the data manager and input into the data management system in batches. In the process of data cleaning, abnormal or missing data will be questioned by the data manager to the researcher and finally solved. The data of this study shall be jointly saved and supervised by the project team and the cooperative units to prevent data loss, modification or improper use.
