研究目的：

课题组采用耳穴疗法（耳穴刺络结合耳穴贴压）治疗失眠疗效较好，前期“耳穴刺络结合贴压对大学生失眠患者睡眠质量及神经内分泌水平影响的研究”结果表明，耳穴刺络结合贴压与单纯贴压均可以改善失眠患者症状，二者疗效相似，耳穴刺络结合贴压干预4周后临床有效率达到86.1%，在3个月随访中，改善中医证候疗效评分方面优于单纯贴压。耳穴刺络结合贴压可促进褪黑素（MT）分泌，降低谷氨酸（Glu）水平，升高γ-氨基丁酸（GABA）水平，降低Glu/GABA比值，纠正二者失衡状态；单纯贴压也促进了MT分泌，降低了Glu水平，但GABA水平也出现降低，Glu/GABA比值未改变。因此耳穴刺络结合贴压在改善中医证候疗效评分、调节睡眠－觉醒周期平衡方面较有优势。但缺乏高质量的循证依据，故本研究以该疗法为治疗的主要措施，以安慰剂作为对照，评价其有效性及安全性，具有较大的临床意义。  

Objectives of Study：

Our previous study found that using auricular acupoint therapy, auricular acupoint bloodletting plus auricular acupressure(AB+AA), can improve insomnia, and "Effect of Auricular Acupoint Bloodletting plus Auricular Acupressure on Sleep Quality and Neuroendocrine Level in College Students with Primary Insomnia: A Randomized Controlled Trial" results showed that both AB+AA and AA can relieve insomnia symptoms, but a stronger cumulative effect and long-term efficacy were observed for AB+AA. The clinical effective rate of reached 86.1% after 4 weeks of AB+AA. In the 3-month follow-up, it was better than simple auricular acupressure in improving the curative effect score of TCM syndrome. AB+AA can promote the secretion of melatonin (MT), reduce the level of glutamate (Glu), increase the level of γ-aminobutyric acid (GABA), reduce the Glu/GABA ratio, and correct the imbalance between them. Therefore, AB+AA had advantages in improving the efficacy score of TCM syndromes and regulating the balance of sleep-wake cycle. However, there is a lack of high-quality evidence-based evidence. So this study takes this therapy as the main treatment measure and placebo as the control to evaluate its effectiveness and safety, which has great clinical significance.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合DSM-5诊断标准；
②年龄在18到65岁之间（含18和65岁），性别不限；
③匹兹堡睡眠指数（Pittsburg Sleep Quality Index，PSQI）评分大于7分，失眠严重程度指数（Insomnia Severity Index，ISI）在8到21分之间（含8和21分）；
④入组前2周内未接受镇静催眠药物治疗、心理治疗或耳穴治疗；
⑤患者对研究知情并签署知情同意书，试验结束后愿意接受随访。

Inclusion criteria

① Meets the diagnostic criteria for the DSM-5.
②Aged 18-65 years, regardless of gender;
③The Pittsburg Sleep Quality Index(PSQI) total score was more than 7 points and the Insomnia Severity Index (ISI) score was more than 8 points but less than 21 points.
④No sedative-hypnotic drug therapy, psychotherapy, or auricular therapy were received within 2 weeks before enrollment;
⑤The patients were informed of the study and signed the informed consent, as well as they were willing to accept follow-up after the end of the study.

排除标准：

①有精神或双相障碍病史的患者和有以下主要睡眠障碍的患者：睡眠呼吸暂停（低通气指数大于15）、不宁腿综合征、周期性肢体运动障碍（PLMSI大于15次/h）、昼夜节律障碍和异睡症；
②抑郁自评量表（Self-Rating Anxiety Scale，SDS）评分大于72分（含72分），焦虑自评量表（Self-Rating Anxiety Scale，SAS）评分大于69分（含69分）；
③妊娠或哺乳期妇女；
④耳部有红肿、溃破、瘢痕及对胶布过敏者；
⑤出血性疾病、凝血功能障碍者、高血糖患者及贫血者；
⑥不愿意佩戴体动记录仪的患者；
⑦拟纳入或已纳入其他临床研究者。

Exclusion criteria：

①Patients with a history of psychiatric or bipolar disorder and those with the following major sleep disorders: sleep apnea (apnea/hypopnea index > 15), restless leg syndrome, periodic limb movement during sleep (PLMS with arousal > 15 per hour), circadian rhythm disorder, and parasomnias;
②The SDS(Self-Rating Anxiety Scale) score was greater than 72 (including 72), and the SAS(Self-Rating Anxiety Scale) score was greater than 69 (including 69)
③Pregnant or lactating women;
④Ear redness, ulceration, scar and allergic to tape at the time of any therapy session;
⑤Patients with hemorrhagic diseases, coagulation disorders, hyperglycemia and anemia;
⑥The patient did not want to take the Actigraphy
⑦Other clinical investigators are planned to be included or have been included.

本研究采用随机对照试验设计，采用中心随机方法，由河北省中医针灸优势病症国际联合中心承担随机分组和数据管理。运用SPSS统计软件，按随机1:1的比例生成随机数字分组表，根据生成的随机数字表将患者分配到治疗组和对照组。随机方案由不参加试验的专门人员管理，施术者在治疗前5～10min（分钟）通过电话知晓此次试验的治疗计划。
