研究目的：

（一）主要目的：调查患者对于颈部矫形器的使用率、使用种类、获得渠道、使用感受和评价。 （二）次要目的：分析影响患者选择或放弃使用颈部矫形器的相关因素。 （三）探索性目的：为改良ALS专用颈部矫形器设计提出针对性建议。  

Objectives of Study：

（1） Main objective: To investigate the usage rate, types, access channels, using feelings and evaluation of cervical orthosis. （2） Secondary objective: To analyze the relevant factors that affect patients' choice or abandonment of using cervical orthosis. （3） Exploratory objective: To make targeted suggestions for improving the design of cervical orthosis for ALS.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥18 岁；
（2）符合1998年修订的El Escorial诊断标准中临床肯定的、很可能的及可能的ALS；
（3）理解力正常者;
（4）接受调查时已出现颈肌无力相关症状的患者;
（5）患者及家属愿意接受本研究涉及的调查内容。

Inclusion criteria

(1) Age ≥ 18 years old;
(2) Meets clinically confirmed, probable and possible ALS in El Escorial diagnostic criteria revised in 1998;
(3) Those with normal comprehension;
(4) Patients with neck muscle weakness at the time of investigation;
(5) Patients and their families are willing to accept the survey contents involved in this study.

排除标准：

（1）任何已知的非ALS相关的运动神经元病；
（2）重大的精神疾病或认知功能障碍的患者；
（3）接受调查时尚未出现颈肌无力相关症状的患者;
（4）不愿或不能参加及完成本研究的患者。

Exclusion criteria：

(1) Any known non-ALS-related motor neuron disease;
(2) Patients with major mental illness or cognitive dysfunction;
(3) Patients without neck muscle weakness related symptoms at the time of investigation;
(4) Patients who are unwilling or unable to participate in and complete the study.
