研究目的：

1. 开发基于移动医疗APP的精神分裂症院外强化管理模式，借助互联网实现症状持续检测、疾病复发预警、院外疾病管理三大目标。 2. 在中、西部地区开展多中心的应用研究，采用随机对照研究，以服药依从性、复发、自我效能感、生活质量、精神症状、认知功能为评价指标分析基于移动医疗APP的精神分裂症院外强化管理模式的应用效果；采用质性研究，分析精神分裂症患者使用精神分裂症院外强化管理模式 App的使用意愿以及优缺点，以期进一步优化软件性能。  

Objectives of Study：

1. Develop a strengthened management model for schizophrenia out of hospital based on mobile medical app, and realize the three goals of continuous symptom detection, disease recurrence early warning and out of hospital disease management with the help of the Internet. 2.Carry out multi center application research in the central and western regions, adopt randomized controlled research, and analyze the application effect of out of hospital strengthened management mode of schizophrenia based on mobile medical app with medication compliance, recurrence, self-efficacy, quality of life, mental symptoms, cognitive function as evaluation indicators; Qualitative research was used toanalyze the willingness, advantages and disadvantages of schizophrenic patients to use the schizophrenic out of hospital enhanced management model app, in order to further optimize the software performance.

药物成份或治疗方案详述：

院外管理的干预主要包括个人中心、疾病监测、疾病管理、康复资讯、问诊小助手五大模块。 个人中心：患者进行个人信息管理和系统管理的模块。患者可设置每日服药时间和服药剂量，勾选提醒服务，系统可定时提醒患者药。 疾病监测：患者可进行每日服药和睡眠情况打卡，及定期量表自评。关联的家属可以对患者的评估结果进行修改和补充。后台可动态监测患者的评估结果，基于大数据，发现患者的异常评估结果。疾病管理：对患者的服药、打卡、复诊、量表填写等需求发送提醒消息；对于疾病监测发现的高风险患者给予疾病可能复发的预警信息；对高风险患者进行个性化干预指导。 康复资讯：包括以视频、文章等形式的有关精神分裂症的资料，患者可分门别类的阅读，观看其中的内容，帮助患者更好的了解疾病、有针对性的管理疾病。 问诊小助手：对于患者存在的疾病相关的常见问题和软件使用障碍等问题，可发起对话，咨询后台工作人员。 

Description for medicine or protocol of treatment in detail：

The intervention of out-of-hospital management mainly includes five modules: personal center, disease monitoring, disease management, rehabilitation information and consultation assistant. Personal center: a module for patients to manage personal information and system management. Patients can set the time and dose of daily medicine, check the reminder service, the system can remind patients to take medicine regularly. Disease monitoring: Patients can take medicine daily and sleep status clock, and regular scale self-evaluation. Related family members can modify and supplement the patient's assessment results. The background can dynamically monitor the evaluation results of patients, based on big data,abnormal patient assessment results were found. Disease management: send reminder messages for patients' needs such as taking medicine, punching in, returning visit and filling in the scale; Early warning of possible recurrence of disease in patients at high risk detected by disease surveillance; Personalized intervention guidance for high-risk patients. Rehabilitation information: including video, articles and other forms of information about schizophrenia, patients can be categorized to read, watch the content, to help patients better understand the disease, targeted management of the disease. Consultation assistant: Initiate a dialogue and consult background staff for common problems related to the disease and software use obstacles 

纳入标准：

①年龄 18-60 岁；
②符合《国际疾病分类》第 10 版(International Classification
of Diseases, ICD-10) 精神分裂症的诊断标准；
③接受过至少3 个月的抗精神病治疗；
④能配合完成调查及相关测试。

Inclusion criteria

① Age 18-60 years old;
② It meets the diagnostic criteria of schizophrenia in the international classification of diseases (ICD-10);
③ Have received antipsychotic treatment for at least 3 months;
④ Be able to cooperate to complete investigation and related tests

排除标准：

①合并严重的或不稳定的心、肝、肾、内分泌等系统躯体疾病；
②合并影响 认知功能的脑器质性病变和其他精神类疾病；
③酒精等物质滥用或依赖者；
④孕妇 、哺乳 期妇女。

Exclusion criteria：

① Complicated with serious or unstable heart, liver, kidney, endocrine and other systemic somatic diseases;
② Combined with brain organic diseases and other mental diseases affecting cognitive function;
③ Alcohol abuse or dependence;
④ Pregnant and lactating women.

盲法：

双盲，分别在干预前、干预结束时、干预结束1年测试两组服药依从性、复发、自我效能感、生活质量、精神症状、认知功能。测试由精神科医生和精神科护士完成，精神科医生和精神病护士对随机分配和干预分组不知情。后台工作人员实施干预，知情分组的情况，但不参与数据收集和分析工作。

Blinding：

Double blind principle. Medication compliance, recurrence, self-efficacy, quality of life, mental symptoms, cognitive function of the two groups were tested before intervention, at the end of intervention and 1 year after intervention. The test was completed by psychiatrists and psychiatric nurses who were unaware of the random assignment and intervention groups. Background staff intervened and informed the group, but did not participate in data collection and analysis.
