研究目的：

本项目将在明确AD危险因素综合干预是否能保护认知功能和降低痴呆发病的基础上，建立和完善一套适合我国独特社会、经济和文化特点的痴呆一级预防方案，并探讨针对脑组织结构性和功能性改变以及遗传易感性为基础的潜在病理学和生物遗传学机制。  

Objectives of Study：

This project aims to investigate whether the multimodal interventions on AD risk factors can protect the cognitive function and reduce the prevalence of dementia. We will further develop a set of dementia prevention programs which are suitable for the unique social, economic, and cultural characteristics of aging population. The underlying pathological and biogenetic mechanisms will be illustrated by structural and functional brain examination and genetic susceptibility.

药物成份或治疗方案详述：

MIND-CHINA将针对山东省阳谷县闫楼镇60-79岁的老年人进行一项随机对照的、多模态的强化干预，干预方式主要包括：（1）生活方式和饮食指导；（2）日常体力活动；（3）社会和益智活动；（4）血管危险因素强化管理。 

Description for medicine or protocol of treatment in detail：

The intervention will target individuals who are aged 60-79 years old and living in the rural communities of Yanlou Town, Yanggu County, western Shandong province. The package of multimodal intervention program consists of different components, in which the Chinese tradition and sociocultural factors will be carefully incorporated, e.g., (1) lifestyle and nutritional guidelines; (2) daily physical activity; (3) social and intellectual activities; and (4) intensive management of vascular risk factors. 

纳入标准：

1、至基线调查时年龄在60-79岁；
2、在山东省阳谷县闫楼镇已长期居住≥1年。

Inclusion criteria

1. Aging 60-79 at the time of baseline investigation;
2. People has been living in Yanlou town, Yanggu county of Shandong province for more than one year.

排除标准：

1、各种原因所致痴呆患者；
2、严重抑郁、精神分裂症等精神疾病患者；
3、因听力及语言障碍无法完成认知量表评估者；
4、因身体原因无法参加基本干预方案（如健步走和广场舞等运动活动）者，包括偏瘫、关节炎、骨质疏松、帕金森病、心功能不全、慢性阻塞性肺疾病、哮喘等疾病患者；
5、拒绝签署知情同意书者。

Exclusion criteria：

1. Patients with dementia;
2. People with mental disorders such as sever depression and schizophrenia;
3. Unable to complete the cognitive examination due to hearing and/or language impairment;
4. People is unable to participate in the basic intervention program (such as walking and square dancing) due to physical issues, including hemiplegia, arthritis, osteoporosis, Parkinson's disease, cardiac dysfunction, chronic obstructive pulmonary disease, asthma and so on;
5. Refuse to sign the inform consent form.

由项目组的统计师采用整群随机分组的方法随机分为3组，使用SAS9.4软件生成随机数种子。

Statistical professionals will use SAS 9.4 software to generate random number seeds and randomly divided all the viiliages in Yanlou Town (52 villiages) into 3 groups using a cluster randomization method.

盲法：

所有受试者及所有参与干预或临床评定的研究人员均不知道谁接受的是何种处理。盲法贯穿于整个社区干预试验实施过程（包括随机数产生、受试行政村的随机分配、干预效果评价、实验过程监督、数据管理、统计分析等过程）,只有当数据清理到质量能接受的水平,方可对适当的人员揭盲。

Blinding：

All subjects and researchers involved in the intervention or clinical evaluation did not know the grouping information. Blinding method will be throughout the intervention trial implementation process (including generation of randomization numbers, the randomized distribution, intervention effect assessment, experiment process monitoring, data management, statistical analysis). Until the data cleansing to quality acceptable level, it will be discovered to the appropriate personnel.

MIND-CHINA项目将采用纸质版的流行病学调查问卷，包含身体检查、认知评估和实验室检查，采用epidata软件将获得的信息录入电子信息系统里。
