研究目的：

1.利用代谢组学和多肽组学，找到利尿剂抵抗心衰和非利尿剂抵抗心衰之间的差异代谢物和多肽片段，并建立利尿剂抵抗的诊断模型； 2.从宏观和微观两个层面揭示心衰利尿剂抵抗后中医证候演变规律。  

Objectives of Study：

1. Using metabolomics and peptidomics, find the differential metabolites and peptide fragments between diuretic-resistant heart failure and non-diuretic-resistant heart failure, and establish a diagnostic model of diuretic resistance; 2. Reveal the evolution law of TCM syndromes after heart failure diuretic resistance from macroscopic and microscopic levels.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.均符合西医心衰诊断标准，利尿剂抵抗患者符合利尿剂抵抗的标准；
2.年龄50-90 岁；
3.美国纽约心脏病学会心功能分级（NYHA）≥Ⅲ级。

Inclusion criteria

1. All meet the diagnostic criteria of Western medicine for heart failure, and patients with diuretic resistance meet the criteria for diuretic resistance;
2. Aged 50-90 years;
3. New York Heart Association Cardiac Function Class (NYHA) >= grade III.

排除标准：

1.无尿（日尿量＜100 ml）、严重肾功能不全（血肌酐＞265.2μmol/L）; 低钾血症（血清钾 <3.5mmol/L）、高钾血症（血清钾>5.5 mmol/L）、严重低血压（卧位收缩压＜90 mmHg，1 mmHg=0.133 kPa）或低血容量；
2.显著影响心室充盈或/和射血功能的心脏结构机械性障碍，如心包积液、缩窄性心包炎、严重的分流性先天性心脏病、严重的瓣膜疾病、肥厚性或限制性心肌病、重度主动脉瓣狭窄、重度肺动脉高压、室壁瘤及心脏肿瘤等；
3.急性心肌梗死、活动性心肌炎、安装起搏器、病窦综合征、洋地黄中毒、Ⅱ度及以上房室传导阻滞、持续性室性心动过速或快速心房纤颤（心室率＞120 次/min）；
4.恶性肿瘤、终末期肾衰竭、严重肝功能不全、自身免疫性疾病、未控制的肾上腺功能中度贫血（血红蛋白＜90 g/L）；
5.妊娠及哺乳期妇女。

Exclusion criteria：

1. Anuria (daily urine volume <100 ml), severe renal insufficiency (serum creatinine >265.2μmol/L); hypokalemia (serum potassium <3.5mmol/L), hyperkalemia (serum potassium >5.5 mmol/L), severe hypotension (recumbent systolic blood pressure <90 mmHg, 1 mmHg=0.133 kPa) or hypovolemia;
2. Mechanical disorders of the heart structure that significantly affect ventricular filling or/and ejection function, such as pericardial effusion, constrictive pericarditis, severe shunt congenital heart disease, severe valve disease, hypertrophic or restrictive cardiomyopathy, severe aortic stenosis, severe pulmonary hypertension, ventricular aneurysm and cardiac tumor, etc.;
3. Acute myocardial infarction, active myocarditis, installation of pacemaker, sick sinus syndrome, digitalis poisoning, second degree and above atrioventricular block, sustained ventricular tachycardia or rapid atrial fibrillation (ventricular rate > 120 beats/min);
4. Malignant tumor, end-stage renal failure, severe hepatic insufficiency, autoimmune disease, moderate anemia of uncontrolled adrenal function (hemoglobin <90 g/L);
5. Pregnant and lactating patients.

1.心衰诊断标准采用《中国心力衰竭诊断和治疗指南2018》及第 15 版《实用内科学》； 2.利尿剂抵抗标准参照《心力衰竭容量管理专家建议》。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

1. The diagnostic criteria for heart failure are based on the "China Heart Failure Diagnosis and Treatment Guidelines 2018" and the 15th edition of "Practical Internal Medicine"; 2. The standard of diuretic resistance should refer to "Expert Recommendations for Volume Management of Heart Failur"
