研究目的：

1.分析KOA动态对线、关节负荷和代谢特点，探讨太极拳对KOA的骨正效应； 2.分析KOA的肌肉质量、力量和代谢及软组织协调性的特点，探讨太极拳对KOA的筋和效应； 3.分析KOA太极拳反应者与非反应者之间生物力学和生物学的筋骨区分指标，探讨太极拳对KOA的骨正筋和效应机制。  

Objectives of Study：

1. Analyze the dynamic alignment, joint load and metabolic characteristics of KOA, and explore the positive bone effect of Taijiquan on KOA; 2. Analyze the characteristics of KOA's muscle mass, strength, metabolism and soft tissue coordination, and explore the tendon and effects of Tai Chi on KOA; 3. To analyze the biomechanical and biological muscle-bone differentiation indexes between KOA Taijiquan responders and non-responders, and to explore the bone-strengthening and effect mechanism of Taijiquan on KOA.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.专业太极拳运动员纳入标准：
（1）武术一级运动员及以上级别；
（2）年龄50-80岁，既往接受过正规太极拳武术套路训练年限≥10年；
（3）身体健康，无下肢关节病史；
（4）太极拳训练过程或者训练结束后无下肢关节疼痛；
（5）退役后每周太极拳练习次数≥3次，每次训练≥60分钟；
（6）自愿参加，并签署知情同意书者。
2. 长期太极拳锻炼者纳入标准：
（1）年龄50-80岁；
（2）每周太极拳练习次数≥3次，每次训练≥60分钟，拳龄≥10年；
（3）太极拳训练过程或者训练结束无下肢关节疼痛；
（4）身体健康，无下肢关节病史；
（5）自愿参加，并签署知情同意书者。
3. 无KOA症状者纳入标准：
（1）经临床医生诊断，无膝骨关节炎症状及诊断者；
（2）年龄50-80岁；
（3）既往无太极拳相关练习经验，且近三个月才开始练习太极拳并掌握国家体育总局创编的太极一段拳（杨氏八式太极拳）；
（4）太极拳训练过程或者训练结束无下肢关节疼痛；
（5）每周太极拳练习次数≥3次，每次训练≥60分钟；
（6）自愿参加，并签署知情同意书者。
4. KOA纳入标准：
（1）符合膝骨关节炎临床诊断标准；
（2）影像学诊断膝关节骨赘形成；
（3）年龄50-80岁；
（4）膝关节X线片K/L分级为1-4级，且能独立完成平地行走至少8米和上、下8级台阶者；
（5）测试前1周内日常活动中膝关节VAS评分10-60mm；
（6）参加试验期间保持原有日常饮食模式，避免较大改变；
（7）自愿参加，并签署知情同意书者。

Inclusion criteria

1. Inclusion criteria for professional Tai Chi athletes:
(1) Martial arts first-level athlete and above;
(2) Aged 50-80 years, have received regular Taijiquan martial arts routine training for >= 10 years;
(3) Good health, no history of lower extremity joint disease;
(4) No lower extremity joint pain during Tai Chi training or after training;
(5) Taijiquan practice times >= 3 times per week after retirement, and each training session is >= 60 minutes;
(6) Those who participated voluntarily and signed the informed consent form.
2. Inclusion criteria for long-term Tai Chi practitioners:
(1) Aged 50-80 years;
(2) The number of Taijiquan exercises per week is >= 3 times, each training session is >= 60 minutes, and the boxing age is >= 10 years;
(3) There is no lower extremity joint pain during or after Tai Chi training;
(4) Good health, no history of lower extremity joint disease;
(5) Those who participated voluntarily and signed the informed consent form.
3. Inclusion criteria for those without KOA symptoms:
(1) Those who have no symptoms and diagnosis of knee osteoarthritis after diagnosis by clinicians;
(2) Aged 50-80 years;
(3) Have no previous experience in Taijiquan practice, and have only started practicing Taijiquan in the past three months and mastered the Taiji Duan Duanquan (Yang's Eight-style Taijiquan) created by the State Sports General Administration;
(4) There is no lower extremity joint pain during or after Tai Chi training;
(5) The number of Tai Chi exercises per week is >= 3 times, and each training session is >= 60 minutes;
(6) Those who participated voluntarily and signed the informed consent form.
4. KOA inclusion criteria:
(1) Meet the clinical diagnostic criteria for knee osteoarthritis;
(2) Imaging diagnosis of knee osteophyte formation;
(3) Aged 50-80 years;
(4) The K/L grading of knee X-ray films is grade 1-4, and the person can independently walk on level ground for at least 8 meters and go up and down 8 steps;
(5) The knee joint VAS score in daily activities within 1 week before the test is 10-60mm;
(6) Maintain the original daily dietary pattern during the trial period and avoid major changes;
(7) Those who participated voluntarily and signed the informed consent form.

排除标准：

1.各种急性、感染性、传染性疾病、内脏功能不全者；
2.神经系统、肌肉骨骼系统其他疾病影响步态者，如中风、骨折、类风湿性关节炎、足底筋膜炎、腰痛等；
3.测试前24小时内使用非甾体类抗炎药或皮质类固醇激素；近2周内曾接受过药物治疗，现代或中医康复方法者；近3个月内曾行关节内类固醇注射或关节镜手术等有手术史；近6个月内曾行关节内透明质酸注射；在1年内准备进行全膝关节置换术者；
4.长期规律进行有氧运动（每周≥3次，持续3个月以上为长期规律）者；
5.严重认知障碍[蒙特利尔认知评测（MoCA<18分）]者；
6.不能配合评估、治疗者。

Exclusion criteria：

1. Various acute, infectious, infectious diseases, visceral insufficiency;
2. Other diseases of the nervous system and musculoskeletal system affect gait, such as stroke, fracture, rheumatoid arthritis, plantar fasciitis, low back pain, etc.;
3. Those who have used non-steroidal anti-inflammatory drugs or corticosteroids within 24 hours before the test; have received drug treatment, modern or traditional Chinese medicine rehabilitation methods within the past 2 weeks; those who have undergone intra-articular steroid injection or arthroscopic surgery within the past 3 months and have a history of surgery; have undergone intra-articular hyaluronic acid injection within the past 6 months; are planning to undergo total knee replacement within 1 year;
4. Long-term regular aerobic exercise (>= 3 times a week for more than 3 months is a long-term regularity);
5. Severe cognitive impairment [Montreal Cognitive Assessment (MoCA < 18 points)];
6. Those who cannot cooperate with evaluation and treatment.

先在SPSS20.0统计软件中依次编写好编号1～82的SPSS数据文件，通过SPSS20.0统计软件的随机数字生成器，随机种子为默认值2000000，产生随数字，对随机数字进行编秩后，按随机数字秩次从小到大进行升序排列，规定1～41归入第一组，42～82归入第二组。根据纳入标准及排除标准将符合条件的82名受试者按入组时间先后顺序依次编号（1～82），并按编号随机分组。

First, write the SPSS data files numbered from 1 to 82 were successively compiled in SPSS20.0 statistical software. The random seeds were generated with the default value of 2000000 through the random number generator of SPSS20.0.
