研究目的：

(1)提供在中国人群中PA的诊断率、规范治疗率及并发症情况等流行病学数据。 (2)用LC-MS/MS法测血清类固醇谱，评价血清中18-羟皮质醇与18-氧皮质醇水平以及评价尿外泌体miRNA作为PA早期诊治的生物学标志物。  

Objectives of Study：

(1) Epidemiological data such as diagnosis rate, standard treatment rate and complications of PA in Chinese population were provided. (2) Serum steroid profiles were measured by LC-MS/MS method to evaluate the serum levels of 18-hydroxycortisol and 18-oxycortisol and urinary exosomal miRNA as biomarkers for early diagnosis and treatment of PA.

纳入标准：

1.根据《原发性醛固酮增多症诊断治疗的专家共识（2020 版）》，ARR>3.7（肾素浓度和醛固酮浓度单位分别为mU/L和ng/dl）的被纳入研究，并且有可用的基线期数据;
2.心脑血管事件定义为：脑出血、脑梗死、短暂性脑缺血发作、心肌梗死、心力衰竭、心绞痛、心律失常、冠状动脉粥样硬化性心脏病;
3.糖尿病定义为：HbA1c≥6.5%，或已经确诊为糖尿病及使用药物、饮食运动控制血糖的患者;
4.肾脏并发症定义为：血清肌酐升高、估算的肾小球滤过率(eGFR)降低，或尿白蛋白排出量增加;
5.血脂异常定义为：血清中胆固醇、甘油三酯水平升高，高密度脂蛋白低于正常水平。

Inclusion criteria

1. According to the Expert Consensus on the Diagnosis and Treatment of primary aldosteronism (2020 version), those with an ARR>3.7 (renin concentration in mU/L and aldosterone concentration in ng/ dL, respectively) were included and baseline data were available;
2. Cardiovascular and cerebrovascular events were defined as cerebral hemorrhage, cerebral infarction, transient ischemic attack, myocardial infarction, heart failure, angina pectoris, arrhythmia, coronary atherosclerotic heart disease;
3. Diabetes mellitus was defined as HbA1c≥6.5%, or patients who had been diagnosed with diabetes and used drugs, diet and exercise to control blood glucose;
4. Renal complications were defined as increased serum creatinine, decreased estimated glomerular filtration rate (eGFR), or increased urinary albumin output;
5. Dyslipidemia was defined as elevated serum levels of cholesterol and triglyceride and lower high-density lipoprotein than normal levels.

排除标准：

1.疑似自主皮质醇分泌者；
2.处于活动性肝脏疾病期，或存在明显的肝功能损害，定义为丙氨酸氨基转移酶（ALT）≥2.5倍正常范围上限；
3.存在精神障碍、不愿意与人交流或存在语言障碍，导致无法充分理解研究方案或配合研究者的；
4.严重的心肺功能不全、恶性肿瘤患者；
5.妊娠状态或正处于哺乳期的女性；
6.无回访数据者。

Exclusion criteria：

1. Suspected voluntary cortisol secretion;
2. Active liver disease stage, or significant liver function damage, defined as alanine aminotransferase (ALT) ≥2.5 times the upper limit of the normal range;
3. Mental disorders, reluctance to communicate with others or language barriers, resulting in inability to fully understand the research protocol or cooperate with the researchers;
4. Patients with severe cardiopulmonary insufficiency and malignant tumors;
5. Pregnant or breastfeeding women;
6. No return data.
