研究目的：

本研究旨在调查在湖南省中南大学湘雅医院行异基因造血干细胞移植术后患口腔慢性移植物抗宿主病的患者情况，并搜集整理患者病历信息，对患者进行问卷调查和口腔临床检查，收集患者血液样本、唾液样本、口腔病理组织样本，探究口腔慢性移植物抗宿主病的不同类型（黏膜病变、唾液腺功能障碍、口腔硬化）和严重程度与血液疾病的对应关系及发病机制；同时，重点关注口腔黏膜下纤维化（OSF）型口腔移植物抗宿主病与血液疾病的关系，探讨分析因移植导致的OSF的临床表现、病理表现、各免疫细胞、细胞因子、核酸水平的变化等与咀嚼槟榔所导致的OSF的区别与联系，并初步探究二者的发病机制有何异同。  

Objectives of Study：

The purpose of this study is to investigate the situation of patients suffering from oral chronic graft-versus-host disease after allogeneic hematopoietic stem cell transplantation in Xiangya Hospital of Central South University in Hunan Province, collect and sort out the patient's medical record information, conduct questionnaire survey and oral clinical examination on patients, and collect blood samples, saliva samples, and oral pathological tissue samples of patients, To explore the corresponding relationship between different types (mucosal lesions, salivary gland dysfunction, oral sclerosis) and severity of oral chronic graft-versus-host disease and blood diseases and the pathogenesis; At the same time, we will focus on the relationship between oral submucous fibrosis (OSF) oral graft-versus-host disease and blood diseases, discuss and analyze the differences and relations between the clinical manifestations, pathological manifestations, changes in immune cells, cytokines, nucleic acid levels of OSF caused by transplantation and the OSF caused by chewing betel nut, and preliminarily explore the similarities and differences in the pathogenesis of the two.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄：18~60（65）周岁
2.因急性淋巴细胞性白血病、复发难治型急性髓系白血病（慢性粒细胞白血病患者是否纳入需咨询血液科老师）于湘雅医院行异基因造血干细胞移植术（Allo-HSCT）治疗后100天以上的患者；
3.移植成功，且原疾病未复发的患者；
4.有口腔表征，且符合慢性移植物抗宿主病（cGVHD）诊断与治疗中国专家共识（2021年版）中口腔cGVHD的诊断标准的患者；
5.签署知情同意书的患者；

Inclusion criteria

1.Age: 18~60 (65)
2.Patients with acute lymphocytic leukemia and relapsed and refractory acute myeloid leukemia (whether chronic myeloid leukemia patients are included in the program needs to consult the hematology teacher) who have been treated with allogeneic hematopoietic stem cell transplantation (Allo HSCT) in Xiangya Hospital for more than 100 days;
3.Patients with successful transplantation and no recurrence of the original disease
4.Patients who have oral manifestations and meet the diagnostic criteria for oral cGVHD in the Chinese expert consensus on the diagnosis and treatment of chronic graft versus host disease (cGVHD) (2021 edition);
5.Patients who signed the informed consent form;

排除标准：

1.孕妇、哺乳期患者；
2.有吸烟、酗酒、咀嚼烟草、咀嚼槟榔史的患者；
3.移植前患有口腔黏膜疾病（包括口腔黏膜下纤维化、口腔扁平苔藓、唾液腺疾病、口腔红斑、口腔鳞状细胞癌）的患者；
4.口腔中有银汞充填物等可能引起口腔扁平苔藓的患者；
5.检查前两周内曾使用过口腔抗真菌药物者；
6.有糖尿病、高血压局部或全身感染等系统性疾病的患者；
7.曾发生过急性移植物抗宿主病的患者。

Exclusion criteria：

1.Pregnant women and lactating patients
2.Patients with a history of smoking, excessive drinking, chewing tobacco and chewing betelnut;
3.Patients with oral mucosal diseases (including oral submucous fibrosis, oral lichen planus, salivary gland diseases, oral erythema, oral squamous cell carcinoma) before transplantation;
4.Patients who may cause oral lichen planus due to amalgam fillings in the mouth;
5.Patients who have used oral antifungal drugs within two weeks before the examination;
6.Patients with systemic diseases such as diabetes, hypertension and local or systemic infections;
7.Patients who have experienced acute graft versus host disease.
