研究目的：

本研究为前瞻性非随机临床研究，拟选择骨性安氏II类、III类需正畸-正颌联合治疗患者下前牙为研究对象，研究辅助增加舌侧骨增量牙周手术的可行性、有效性和长期安全性。计划完成各10例严重骨性安氏II类、III类错牙合畸形患者正畸-正颌联合治疗中配合牙周舌侧骨增量，以降低这两类错牙合畸形患者治疗中舌侧牙周软硬组织的风险、从而最大可能预防规避骨性II类、Ⅲ类错牙合畸形患者正畸-正颌序列治疗后出现牙周支持组织丧失引起的美学甚至功能风险。  

Objectives of Study：

This study would be performed as a prospective nonrandomized controlled trial which aim to study the feasibility, effectiveness and long-term safety of lingual augmented corticotomy periodontal surgery of lower anterior teeth in patients with severe skeletal class II and III patients requiring orthodontic-orthognathic treatment.It is planned to complete the 10 patients with severe skeletal class II and III malocclusion respectively in order to reduce the risk of lingual periodontal soft and hard tissue in the presurgical orthodontic procedure. Thus, the aesthetic and functional risks caused by loss of periodontal supportive tissue in patients with skeletal class II and III malocclusion after orthodontic and orthognathic sequential treatment should be prevented.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄18～40岁，全身健康；②骨性安氏II 、III类错牙合畸形，已知情同意接受正畸-正颌联合治疗；③牙周健康或已控制的牙龈炎、轻度牙周炎患者；④锥形束CT影像学观察测量下前牙区唇侧或舌侧牙槽骨厚度小于0.5mm或下前牙根尖区基骨唇舌向宽度小于牙根宽度；⑤临床检查前牙区根形暴露明显，可扪及牙根凸起；⑥结合正畸需要牙齿移动的距离和范围，优先对骨性安氏II 、III类术前正畸去代偿过程中，需要下前牙切缘舌向回收或唇倾移动大于3mm的正畸治疗牙配合舌侧牙周骨增量手术。⑦同意参加本项研究者。

Inclusion criteria

1. Aged 18-40 years, systemic healthy;
2. Skeletal class III malocclusion with the need of orthodontic and orthognathic treatment;
3. Periodontally healthy, or controlled gingivitis or mild periodontitis;
4. The thickness of labial alveolar bone in upper and lower anterior teeth is less than 0.5mm or the width of labial-lingual base bone in apical region of lower anterior teeth is less than the width of root by cone-beam CT imaging observation;
5. Clinical examination showed that the root appearance of upper and lower anterior teeth is obviously exposed, and the root protrusion could be palpable;
6.According to the tooth movement required by orthodontic treatment, the incisal margin of lower anterior teeth need to move more than 3mm labially or lingually.
7. Agreement in participating in the research.

排除标准：

①孕妇或哺乳期妇女；②患有肿瘤及全身性严重疾病者；③口腔及颌面部急性炎症；④中、重度牙周炎患者或未控制的牙龈炎、轻度牙周炎患者；⑤先天性唇腭裂患者，颜面及牙颌发育明显不对称患者；⑥重度吸烟者(>10支/天)；⑦有精神病疾患或其他原因不能配合治疗者。⑧上颏棘与根尖距离小于2mm，舌侧骨增量手术风险较大者。⑨凝血功能异常，放化疗患者，3年内服用抗骨质疏松药物等

Exclusion criteria：

1. Pregnant or lactating women;
2. Systemic disease or tumors;
3. Acute inflammation of oral cavity and maxillofacial region;
4. Patients with moderate to severe periodontitis or uncontrolled gingivitis or mild periodontitis;
5. Cleft lip and palate or maxillofacial abnormalities;
6. Smoking addicts (>10 cigarettes per day);
7. Patients with mental illness or other reasons who cannot cooperate with the treatment.
8. The distance between the upper mental spine and the root tip is less than 2mm, and the risk of lingual bone increment surgery is relatively high;
9. Coagulation abnormalities, patients with chemoradiotherapy, taking anti-osteoporosis drugs within 3 years, etc

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

病例记录表：研究者根据患者的原始纸质观察记录、患者影像学资料，将数据及时、完整、正确、清晰地录入病例记录表。录入方式采用双人双机录入。 电子采集和管理系统：原始数据在按要求完成数据录入和核查后，按编号的顺序归档保存，并填有检索目录等，以备查考。电子数据文件包括数据库、编码本和说明文件等，分类保存，并有多个备份保存于不同加密硬盘上，妥善保存，防止损坏。在任何学术及其它条件下，受试者信息均受到严格保密。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Case Record Form: According to the original paper observation records of patients, the data were timely, complete, correct and clear input into the case record form. The input mode is two-person and two-machine. Electronic Data Capture: After completing data entry and verification as required, the original data shall be filed and stored in serial order, and filled with retrieval catalogue for future reference. Electronic data files, including databases, code books and description files, are stored in different categories, and multiple backups are stored on different encrypted hard disks to prevent damage. Subject information will be kept strictly confidential under all academic and other conditions.
