研究目的：

探讨个性化呼吸末正压（PEEP）在预防体外循环心脏手术后患者发生肺不张的应用效果  

Objectives of Study：

To investigate the effect of individualized positive end respiratory pressure (PEEP) in preventing atelectasis in patients undergoing cardiopulmonary bypass

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥18岁；②体外循环下行心内直视手术（冠状动脉搭桥术、心脏瓣膜置换术、A型主动脉夹层、法洛四联症、心房粘液瘤等）；③预计经口气管插管行呼吸机辅助通气时间≥48h者。

Inclusion criteria

① Age ≥18 years old; ② Open heart surgery under cardiopulmonary bypass (coronary artery bypass grafting, heart valve replacement, type A aortic dissection, tetralogy of fallot, atrial myxoma, etc.); ③ The estimated time of ventilator-assisted ventilation through airway intubation was ≥48 hours.

排除标准：

①平均动脉压＜60mmHg（小中等量血管活性药物使用情况下）或者过去2小时内使用血管活性药物的剂量不断增加；②气管切开；③既往有中重度COPD或肺气肿病史；④肺大泡；⑤气胸；⑥颅内高压；⑦术后使用ECMO或IABP辅助治疗者；⑧；严重肺动脉高压者；⑨拒绝参加本研究者。

Exclusion criteria：

① mean arterial pressure < 60mmHg (under small and medium doses of vasoactive drugs) or increasing doses of vasoactive drugs in the past 2 hours; ② tracheotomy; ③ Have a history of moderate to severe COPD or emphysema; (4) pulmonary bullae; 5 pneumothorax. ⑥ Intracranial hypertension; (7) Postoperative adjuvant therapy with ECMO or IABP; Today; Patients with severe pulmonary hypertension; ⑨ Refused to participate in the study.
