研究目的：

确定外周血自然杀伤细胞浓度及其治疗对不明原因RIF患者妊娠结局的影响，提高RIF患者的活产率。  

Objectives of Study：

To determine the effect of peripheral blood natural killer cell concentration and its treatment on pregnancy outcome of patients with unexplained RIF, to improve the live birth rate of patients with RIF.

药物成份或治疗方案详述：

本研究回顾性分析不明原因反种植失败(URIF)患者外周血自然杀伤细胞(pNKC)检查和治疗结局，根据其在外周血淋巴细胞的占比分为两组：正常pNKC组(<18%)，高pNKC组(≥18%)，行胚胎移植后观察两组患者的胚胎种植率、临床妊娠率、流产率及活产率，以确定不同pNKC浓度对URIF患者妊娠结局的影响。后期将两组分为口服糖皮质激素与对照组(不使用糖皮质激素)，观察胚胎移植后患者的胚胎种植率、临床妊娠率、流产率及活产率，明确糖皮质激素对是否可改善URIF的妊娠结局，妊娠结局的改善是否与pNKC的浓度有关。 

Description for medicine or protocol of treatment in detail：

In this study, peripheral blood natural killer cells (pNKC) were examined in patients with unexplained implantation failure (uRIF). According to their proportion in peripheral blood lymphocytes, they were divided into two groups: normal pNKC group (< 18%) and high pNKC group (>= 18%). After embryo transfer, the embryo implantation rate, clinical pregnancy rate, abortion rate and live birth rate of the two groups were observed, to determine the effect of different pNKC concentrations on pregnancy outcome in uRIF patients. Later, the two groups were divided into oral glucocorticoids and control group (without glucocorticoids). The embryo implantation rate, clinical pregnancy rate, abortion rate and live birth rate of patients after embryo transfer were observed to determine whether glucocorticoids can improve the pregnancy outcome of uRIF and whether the improvement of pregnancy outcome is related to the concentration of pNKC. 

纳入标准：

1. 年龄20-40岁；
2. 诊断为不明原因反复种植失败；
3. 男女双方染色体正常。

Inclusion criteria

1. Aged 20 to 40 years;
2. Diagnosed as URIF;
3. The chromosomes of spouse are normal.

排除标准：

1. 患者拒绝签署知情同意书；
2. 黄体功能不全、甲状腺功能异常、糖尿病、催乳素腺瘤及自身免疫性疾病患者；
3. B超下可见的双侧输卵管积水未行结扎患者；
4. 生殖道畸形、慢性子宫内膜炎患者；
5. 合并子宫内膜异位症，子宫肌瘤，宫腔粘连 (包括既往宫腔粘连分离术) 患者；
6. 血栓前状态患者；
7. 移植当月行内膜搔刮，宫腔灌注患者；
8. 吸烟、酗酒、饮用过量咖啡、滥用药物等不良嗜好患者；
9. 不良环境因素，例如有害化学物质的过多接触、放射线的过量暴露等；
10. 对药物泼尼松有严重速发性过敏。

Exclusion criteria：

1. Patients refused to sign the informed consent form;
2. Patients with luteal insufficiency, abnormal thyroid function, diabetes, prolactinoma and autoimmune diseases;
3. Patients with bilateral hydrosalpinx visible under ultrasound without ligation;
4. Patients with genital tract malformations and chronic endometritis;
5. Patients with endometriosis, uterine fibroids, intrauterine adhesions (including previous intrauterine adhesion separation)
6. Patients with prethrombotic state;
7. Patients weith endometrial scraping and intrauterine perfusion in the embryo transfer cycle;
8. Patients with bad habits such as smoking, alcohol abuse, excessive coffee drinking, drug abuse, etc.
9. Unfavorable environmental factors, such as excessive exposure to harmful chemical substances, excessive exposure to radiation, etc.
10.Severe immediate allergy to the drug prednisone.
