研究目的：

建立一项随机对照研究以评估基于超声检测的强化胰岛素教育对于皮下脂肪增生患者血糖控制的影响。  

Objectives of Study：

To establish a randomized controlled trial to evaluate the effect of intensive insulin education based on ultrasound testing on glycemic control in patients with subcutaneous adipose hyperplasia.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄≥18周岁，男女不限；
2. 每日至少进行一次胰岛素皮下注射治疗已达1年以上者；
3. 血糖控制不佳（HbA1c＞7.5%）；
4. 自愿加入本调查研究，并签署相关知情同意书。

Inclusion criteria

1. Age >= 18 years, male or female;
2. Patients who have received subcutaneous injection of insulin at least once a day for more than 1 year;
3. Poor blood glucose control (HbA1c > 7.5%);
4. Voluntarily participate in the study and sign relevant informed consent forms.

排除标准：

1. 合并急性感染、酮症酸中毒等严重并发症；
2. 目前正在使用如胰高糖素样肽-1(Glucagon-like peptide-1，GLP-1)受体激动剂等需要腹部皮下注射药物治疗者；
3. 已知存在注射部位肿块者，如脂肪瘤、皮质腺囊肿、皮下囊肿等；
4. 合并如银屑病、湿疹等腹部皮肤炎症性病变；
5. 已知或可能对医用超声耦合剂过敏；
6. 无法完成一般信息采集或临床体格检查；
7. 既往有腹部手术治疗史，存在明显疤痕者；
8. 频繁的或者最近出现严重低血糖；
9. 妊娠期妇女；
10. 合并精神疾病和认知功能障碍及生活无法自理。

Exclusion criteria：

1. Acute infection, ketoacidosis and other serious complications;
2. Currently using glucagon-like peptide-1 (GLP-1) receptor agonists and other drugs requiring subcutaneous injection into the abdomen;
3. Known mass at injection site, such as lipoma, cortical gland cyst, subcutaneous cyst, etc.;
4. Complications such as psoriasis, eczema and other abdominal skin inflammation;
5. Known or possible allergy to medical ultrasound couplings;
6. Unable to complete general information collection or clinical physical examination;
7. Patients with a history of previous abdominal surgery and obvious scars;
8. Frequent or recent episodes of severe hypoglycemia;
9. Pregnant women;
10. Complicated with mental illness and cognitive dysfunction and unable to take care of themselves.
