通过使用一次性支气管镜，探索宏基因组二代测序技术监测下呼吸道病原微生物，在使用抗生素后下呼吸道病原微生物清除及后期疾病转归情况  

Objectives of Study：

By using disposable bronchoscopy, the next generation of metagenomic sequencing technology is explored to monitor the pathogenic microorganisms of the lower respiratory tract, the clearance of pathogenic microorganisms in the lower respiratory tract after the use of antibiotics, and the post-disease outcome

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄>18周岁，明确诊断社区获得性肺炎，且符合中国成人社区获得性肺炎诊断和治疗指南（2016年版）SCAP诊断标准。
气管插管小于24小时。
愿意首次自费送检肺泡灌洗液mNGS检测。

Inclusion criteria

Age > 18 years, definitive diagnosis of community-acquired pneumonia, and meet the SCAP diagnostic criteria of the Chinese Guidelines for the Diagnosis and Treatment of Community-Acquired Pneumonia in Adults (2016 Edition).
Endotracheal intubation is less than 24 hours.
Willing to send alveolar lavage fluid mNGS for the first time at their own expense.

排除标准：

1）支气管扩张症
2）活动性肺结核患者
3）阻塞性肺炎
4）发病前2周有住院治疗史，本次感染不能除外医院获得性感染者
5）入组前已经住院或行机械通气治疗≥2天的患者
6）免疫功能抑制宿主：包括HIV阳性患者、粒细胞缺乏持续10天以上、骨髓干细胞移植、实体器官移植、近期糖皮质激素冲击治疗、免疫抑制剂治疗、伴发实体肿瘤接受放化疗治疗等患者。
7）不能理解、执行调查方案的患者
8）不能行纤支镜灌洗检查

Exclusion criteria：

1) Bronchiectasis
2) Patients with active tuberculosis
3) Obstructive pneumonia
4) There is a history of hospitalization 2 weeks before the onset of illness, and this infection cannot exclude hospital-acquired infections
5) Patients who have been hospitalized or undergone mechanical ventilation therapy for ≥ 2 days before admission
6) Immunosuppressive hosts: including HIV-positive patients, granulocyte deficiency lasting more than 10 days, bone marrow stem cell transplantation, solid organ transplantation, recent glucocorticoid shock therapy, immunosuppressant therapy, concomitant solid tumors undergoing chemoradiotherapy and other patients.
7) Patients who cannot understand and implement the investigation plan
8) Fiber branchoscopic lavage examination cannot be performed
