研究目的：

本项目使用仪器检查老年人各项体检常规指标，获得其健康状况数据。同时开展老年人中医体质辨识，并进行CGA工具评估，为每一位老人建立健康档案。目的是由知名专家指导，依据老人们检查后的数据、体质辨别情况及CGA评分情况，采用中医药在治未病、疾病治疗和康复中的相关手段，对老人们制定个体化干预方案，团队化进行健康管理。  

Objectives of Study：

The health condition data of the elderly will be obtained after physical examination, and at the same time, traditional chinese medicine (TCM) constitution identification of the elderly as well as comprehensive geriatric assessment tools will be evaluated to establish health records for each elderly. Our aim is to realize individualized intervention plan for each elderly and accomplish team health management by adopting TCM in the prevention, treatment and rehabilitation of existing diseases, guided by well-known experts, personal examination data, physical fitness discrimination and CGA score of the elderly.

纳入标准：

将意愿参与本课题并完成签署知情同意书的老人进行纳入。

Inclusion criteria

The elderly who are willing to participate in this study and has completed signing the informed consent form will be included.

排除标准：

1.慢性疾病临终状态者； 2.合并免疫缺陷病、严重心血管疾病、终末期肾病、恶性肿瘤者；3.意识障碍或精神异常，不能配合本研究者；4.因各种原因中途退出者；5.拒绝或不能执行治疗方案者。

Exclusion criteria：

1. Terminal patients with chronic diseases; 2. Patients with immunodeficiency disease, severe cardiovascular disease, end-stage renal disease and malignant tumor; 3. Patients with impaired consciousness or mental disorder, unable to cooperate with the researcher; 4. Quit midway due to various reasons; 5. Refuse or fail to follow the treatment plan.
