研究目的：

1、探索氨甲环酸联合用于腰椎后路固定融合手术的用法用量，观察氨甲环酸应用后输血率的变化； 2、观察腰椎后路固定融合术后应用利伐沙班后VTE发生率、引流量和血肿发生率等。  

Objectives of Study：

1. Explore the usage and dosage of tranexamic acid for lumbar posterior fixation fusion surgery, and observe the change of transfusion rate after application of tranexamic acid; 2. Observe the incidence of VTE, drainage and hematoma incidence after the application of rivaroxaban after lumbar posterior fixation fusion.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄大于18 周岁且小于80 周岁的成年人；
2.诊断为腰椎椎管狭窄症或腰椎滑脱症且需进行后路减压及椎间融合术的患者；
3.麻醉风险等级评分ASA1-3级。

Inclusion criteria

1. Adults aged 18 to 80 years;
2. Patients diagnosed with lumbar spinal stenosis or lumbar spondylolisthesis requiring posterior decompression and interbody fusion;
3. Anesthesia Risk Rating Scale ASA1- Level .

排除标准：

1、合并PE或DVT的患者；
2、明显肝病 (如乙肝、丙肝、急性肝炎, 慢性活动性肝炎, 肝硬化)的患者；
3、肌酐清除率 ＜30 mL/min的患者；
4、活动性出血或出血高风险的患者；
5、收缩压 ＞180 mmHg 或舒张压 ＞110 mmHg，或血压控制不理想的患者；
6、未采取避孕措施可能怀孕者、孕妇或哺乳期妇女；
7、需同时应用CYP3A4强效抑制剂者；
8、后天色觉障碍的患者；
9、使用凝血酶的患者；
10、有抽搐病史的患者；
11、有血栓形成倾向的房颤的患者、支架植入术后的患者、安置心脏起搏器的患者等；
12、要求行脊柱微创手术的患者；
13、长期口服抗血小板药物的患者；
14、长期服用二甲双胍的患者；
15、Hb＜90g/L；16、对氨甲环酸或利伐沙班过敏的患者。

Exclusion criteria：

1. Patients with PE or DVT;
2. Patients with obvious liver diseases (such as hepatitis B, hepatitis C, acute hepatitis, chronic active hepatitis, cirrhosis);
3. Patients with creatinine clearance <30 mL/min;
4. Activity Patients with a high risk of bleeding or bleeding;
5. Patients with systolic blood pressure> 180 mmHg or diastolic blood pressure> 110 mmHg or unsatisfactory blood pressure control;
6. Pregnant women, pregnant women or breast-feeding women who have not taken contraceptive measures; CYP3A4 potent inhibitors;
7. Patients with acquired color vision disorders;
8. Patients with thrombin;
9. Patients with a history of convulsions;
10. Patients with thrombophilic atrial fibrillation, stent implantation Patients, patients with cardiac pacemakers, etc.;
11. Patients who require minimally invasive spine surgery;
12. Patients with long-term oral antiplatelet drugs;
13. Patients with long-term metformin use;
14. Hb < 90g/L;
15. Patients with tranexamic acid or rivaroxaban allergy.

患者入院后，首先进行Autar评分，根据评分结果划分至低危组、中高危组，然后预先由无利益关系、且对实验不知情的第三方人员使用随机数字表产生随机数字，然后随机标记信封，再抽取信封将该患者分入不同的亚组。

After the patients were admitted to the hospital, the Autar score was first performed. According to the results of the scoring, the patients will be assigned to the low-risk group and the middle-high-risk group. The tube bed physician then take the envelope to divide the patients into different subgroups.
