建立前瞻性上消化道癌筛查队列 评估上消化道癌筛查的效果 构建人群筛查队列数据库和大型生物样本分析库  

Objectives of Study：

1. Establish a multi-center large upper gastrointestinal cancer screening cohort in China; 2. Evaluate the feasibility of the upper gastrointestinal cancer screening in China; 3. Build up a large database and biobank for the cohort subjects.

纳入标准：

选取具有良好的筛查项目工作基础和肿瘤登记工作基础的人群开展，要求目标人群相对稳定，涵盖高发区和低发区。以村/社区为单位，在高发区建立6万人群的筛查队列，在非高发区建立8万人群的筛查队列，筛查组和对照组1:1分配，并开展队列随访。项目点共7个，分配如下：上消化道癌高发区3个，河南林县、河北磁县和甘肃武威各20000人；非高发区4个，江苏射阳、河南罗山、哈尔滨、长沙市各20000人。研究对象分为筛查组和对照组，两组人群选自同一地区，以村/社区为单位，抽取筛查组和对照组人群，对筛查组开展筛查，对照组进行随访。两组研究对象的入选标准是：40-69岁当地户籍居民。既往无癌症史，一般状况良好，近3年未参加过内镜筛查，自愿参与本项目。

Inclusion criteria

A total of seven cities/counties would be selected as the screening centers, covering areas with high incidence of upper gastrointestinal cancer and middle/low incidence rates. Multi-center cluster based randomization would be used. Cases and controls will be 1:1 matched. In areas with high incidence of upper gastrointestinal cancer, 60 000 subjects will be enrolled. In areas with middle/low incidence of upper gastrointestinal cancer, 80 000 subjects will be enrolled. The areas with high risk of uppergastrointestinal cancer are located at: Linzhou County of Henan Province, Cixian County of Hebei Province, Wuwei County of Gansu Province. The areas with middle/low risk of uppergastrointestinal cancer are located at: Sheyang County of Jiangsu Province, Luoshan County of Henan Province, Harbin City of Heilongjiang Province. The inclusion criteria includes: local residents aged 40-69, no history of cancer, no history of endoscopic examination in the latest 3 years, willing to accept endoscopic examination.

排除标准：

明确诊断为食管癌或胃癌或患其他癌症的患者；
患其他严重疾病无法耐受内镜检查；
年龄在40-69岁范围之外者；
正在进行癌症治疗者；
近3年内做过食管镜/胃镜者；
不愿签署知情同意书者。

Exclusion criteria：

1. Subjects with history of cancer or severe diseases who are tolerate endoscopic examination;
2. Subjects who are not willing to take endoscopic examination;
3. Men and women who at the time of randomization are <40 or >69 years of age;
4. Individuals currently undergoing treatment for cancer;
5. Individuals who had uppergastrointestinal endoscope in the past three years;
6. Individuals who are unwilling to sign the consent form.

在受试对象入组前完成。7个筛查现场的工作人员上报参加村/社区的名单。由中国医学科学院肿瘤医院统计师编写SAS程序，以村为单位整群随机划分成两组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Before the enrollment of the subjects, the working staff from seven screening centers provide the list of the participating villages/communities of their areas. The statisticians from Cancer hospital, Chinese Academy of Medical Sciences use the SAS software to accomplish automatic group randomization.
