研究目的:

心率变异性(heartrate variability,HRV)是指连续心动周期之间的变化,反映了自主神经对心脏窦房结功能的调节过程。通过线性和非线性分析,HRV可以定量地反映心脏迷走神经和交感神经之间的平衡。HRV测量过程无创、操作简便、重复性好,已成为评价自主神经系统功能最常用的无创方法之一。 术中低体温是指接受手术的患者术中核心体温低于36°C。术中低体温在没有主动加热的手术患者中很常见,可能导致严重并发症,包括凝血障碍、伤口感染、苏醒延迟等。术中核心体温下降的第一阶段是由于麻醉诱导后体温从核心重新分布到外围。体温下降的第二阶段通常是由于身体向环境的热量散失超过代谢热能的产生。与术中低体温有关的因素,尤其是与第一阶段体温再分配有关的因素,如患者的BMI、年龄和基础代谢水平等,都与HRV相关。同时,HRV作为一种简单易测量的指标,在环境科学研究中经常被用作评价人体热感觉和热舒适性的重要指标。因此,在本研究中,我们将观察麻醉诱导前5-10分钟HRV与术中低体温发生之间的相关性。  

Objectives of Study:

Heart rate variability (HRV) refers to the variation between successive cardiac cycles, which reflects the regulation process of autonomic nerve on the function of cardiac sinoatrial node. HRV can be quantitatively evaluated to reflect the balance between cardiac vagus nerve and sympathetic nerve through linear and nonlinear analysis. The HRV measurement process is noninvasive, easy to operate, and has good repeatability, and has become one of the most commonly used noninvasive methods for evaluating the function of autonomic nervous system. Intraoperative hypothermia refers to the intraoperative core body temperature of patients undergoing surgery less than 36 °C. Intraoperative hypothermia is common in surgical patients without active heating and can lead to serious complications including coagulation disorders, wound infection, delayed recovery, and thermal discomfort. The first stage of intraoperative core temperature drop is due to the redistribution of body temperature from the core to the periphery after induction of anesthesia. The second stage of the body temperature decline is often caused by the body's heat loss to the environment exceeding metabolic heat generation. The factors related to intraoperative hypothermia, especially the factors related to the redistribution of body temperature in the first stage, like the patient's BMI, age and the basal metabolic level, etc., which are all correlated with HRV. Meanwhile, HRV, as a simple and easy-to-measure indicator, is often used as an important indicator to evaluate the human body's thermal sensation and thermal comfort in environmental science research. Therefore, in this study, we will observe the correlation between 5-10 min HRV before anesthesia induction and the occurrence of intraoperative hypothermia.

纳入标准:

1.18岁或以上
2.接受腹部大手术且预计持续2-4小时
3.在全身麻醉下进行择期手术的患者
4.知情同意

Inclusion criteria

1.18 years of age or older
2.Patients receiving major abdominal surgery expected to last 2-4 hour
3.Patients who will undergo elective surgery with general anaesthesia
4.Informed consent

排除标准:

1.无窦性心律或正服用抗心律失常药物
2.近期有发热史或正服用已知会影响体温的药物
3.患严重心脑血管疾病或肝肾功能不全
4.有出血或心肌梗死的特殊风险(由主治麻醉医师确定)
5.术中出血>800毫升或术中循环不稳定(由主治麻醉医师确定)
6.主治麻醉医师认为加入试验不符合患者的最佳利益

Exclusion criteria:

1.Patients were not in sinus rhythm or on anti-arrhythmic medications
2.Patients with recent history of fever or currently taking a medication that is known to influence body temperature
3.Patients with severe cardiovascular and cerebrovascular diseases or liver or kidney dysfunction
4.Patietns were at special risk for bleeding or myocardial infarction (as determined by the attending anesthesiologist)
5.Patients with intraoperative bleeding > 800 ml, or intraoperative circulatory instability (as determined by the attending anesthesiologist)
6.Attending anesthesiologist does not believe participation of the patient is in their best interest
