白内障在大于60岁人群中发病率超过80%,随着老龄化进程的加快,白内障手术量逐年增加。白内障手术大部分于日间病房收治,局麻下进行操作,患者精神紧张,疼痛感强,对手术操作及患者围术期安全带来极大危害。少部分患者接受镇静麻醉,主要使用丙泊酚等镇静药物,但对于呼吸、循环抑制较为明显。因此,找到适合白内障手术基础麻醉药物,在提供舒适医疗的同时避免循环和呼吸抑制并发症,提高围术期患者安全,变得尤为重要。 |

Objectives of Study: |
The incidence rate of cataract in people over 60 years old is more than 80%. With the acceleration of the aging process, the number of cataract surgery increases year by year. Cataract surgery is mostly operated under local anesthesia. The patients are nervous and have a strong sense of pain, which brings great harm to the operation and perioperative safety of patients. A small number of patients received sedative anesthesia, mainly using propofol and other sedative drugs, but the respiratory and circulatory inhibition was obvious. Therefore, it is particularly important to find the basic anesthetic drugs suitable for cataract surgery, to provide comfortable medical care while avoiding circulatory and respiratory suppression complications, and to improve the safety of perioperative patients. |

瑞马唑仑是新一代的苯二氮卓类镇静药物,起效迅速,主要通过非特异性血浆酯酶代谢,代谢产物无药理活性,而且有特异性拮抗药-氟马西尼,可使患者在90秒内迅速苏醒。配合小剂量瑞芬太尼静脉给药,可缓解球后注射睫状神经节阻滞时的疼痛,从而减轻对血压、心率的波动,避免疼痛造成体动对于手术操作的干扰。 |

Description for medicine or protocol of treatment in detail: |
Remimazolam is a new generation benzodiazepine sedative drug. It takes effect quickly, mainly through non-specific Plasma Esterase metabolism. The metabolites have no pharmacological activity, and there is a specific antagonist - flumazenil, which can make the patient wake up quickly within 90 seconds. Combined with intravenous administration of small dose remifentanil, it can relieve the pain caused by posterior bulbar injection of ciliary ganglion block, thus reducing the fluctuation of blood pressure and heart rate, and avoiding the interference of body movement caused by pain to the operation. |

排除标准: |
①合并严重循环、呼吸系统基础疾病;②困难气道;③肝、肾功能不全;④术前简易精神状态检查量表(MMSE)评分符合痴呆;⑤精神障碍;⑥听力障碍;⑦长期服用镇静药;⑧不能配合。 |

① Complicated with severe circulatory and respiratory basic diseases; ② Difficult airway; ③ Liver and kidney dysfunction; ④ Preoperative mini mental state examination scale (MMSE) scores were consistent with dementia; ⑤ Mental disorder; ⑥ Hearing impairment; ⑦ Long term use of sedatives; ⑧ Unable to cooperate. |
