研究目的：

本研究以格林模式理论框架为指导，制定并实施有针对性的社区健康教育，并对干预效果进行评价，以期提高兰州市社区空巢老年糖尿病患者自我管理能力，改善患者的血糖控制水平，延缓或避免糖尿病并发症的发生。  

Objectives of Study：

This study develops and implements targeted community health education and evaluates the effect of intervention, with a view to improving the self-management ability of the elderly diabetic patients with empty nest in Lanzhou, improving the blood sugar control level of the patients, and delaying or avoiding the occurrence of diabetes complications.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥60周岁且为兰州市社区常住居民；
2.无子女、保姆照顾，一人独居或夫妻同住。
3.意识清楚，精神、认知功能正常，沟通无障碍。

Inclusion criteria

1. Aged over 60 years and a resident of the Lanzhou community;
2. No children, nanny care, one person living alone or husband and wife living together.
3. Clear consciousness, normal mental and cognitive function, and no communication impediments.

排除标准：

1. 合并糖尿病严重并发症、精神异常、存在听力、意识、语言障碍等，不能配合者；
2. 因各种原因导致身体状况较差者、卧床甚至行动不便者。

Exclusion criteria：

1. With severe complications of diabetes mellitus, mental abnormalities, hearing, consciousness, language impairment, etc., unable to match;
2. Poor physical condition, bedridden and even inconvenient for various reasons.

按照经济水平将兰州市五个区进行排序产生2个集群（高经济水平区、低经济水平区）。第三个评估员利用随机数字表法随机在2个集群中选择2个区域，使用相同的方法在每个区域中各选择1个社区作为试验组和对照组。通过横幅或通知进行招募研究对象，研究者可根据纳入和排除标准筛选他们，以确定是否有资格参加，最终每组随机分配50名患者，他们自愿加入并签署知情同意书，但研究开始之前不知道他们将被分配到哪一组。
