研究目的：

营养管理是改善腹膜透析（PD）患者营养状态、防治并发症、延缓疾病进展的有效手段，本研究希望将微信平台结合个体化营养管理应用于PD患者当中，通过PD护理专科门诊为患者制定具体可行的个体化营养管理方案，并借助微信对患者进行有效监督和指导，改变患者营养管理观念薄弱、居家营养管理依从性低的现状，提高患者肾脏病饮食态度和行为依从性，改善患者的营养状态和生活质量。同时总结该方式的应用效果，为临床营养管理工作提供借鉴和参考。  

Objectives of Study：

Nutrition management is an effective means to improve the nutritional status of peritoneal dialysis (PD) patients, prevent and treat complications, and delay disease progression. This study hopes to apply wechat platform and individualized nutrition management to PD patients, formulate specific and feasible individualized nutrition management plans for patients through the PD nursing specialty outpatient department, and effectively supervise and guide patients through wechat, so as to change the weak concept of nutrition management The low compliance of home nutrition management can improve the dietary attitude and behavior compliance of patients with renal disease, and improve the nutritional status and quality of life of patients. At the same time, the application effect of this method was summarized to provide reference for clinical nutrition management.

纳入标准：

1.自愿参与本研究，意识清晰，认知正常；2.规律PD≥3个月，年龄18-75岁；3.无其他重大器官或组织损伤；4.会使用微信。

Inclusion criteria

1. Voluntarily participate in this study, with clear consciousness and normal cognition; 2. Regular PD ≥ 3 months, age 18-75 years; 3. No other major organ or tissue damage; 4. Can use wechat.

排除标准：

1.有严重心脑血管疾病；2.合并自身免疫性、感染性疾病；3.合并系统性皮肤疾病；4.合并恶性肿瘤；5.沟通障碍，无法配合研究；6.不会使用微信。

Exclusion criteria：

1. Serious cardiovascular and cerebrovascular diseases; 2. Complicated with autoimmune and infectious diseases; 3. Complicated with systemic skin diseases; 4. Complicated with malignant tumor; 5. Communication barrier, unable to cooperate with the study; 6. Can not use wechat.
