研究目的：

目前我国自体动静脉内瘘（AVF）术后护理模式多侧重院内常规护理干预，但由于患者出院后可因护理知识缺乏、自我管理意识薄弱、门诊随访依从性差等因素，造成AVF成熟不良、AVF相关并发症的发生，从而影响血液透析治疗的顺利进行。本次研究希望通过开展医护一体化的血管通路门诊在AVF术后延续性护理中的应用，帮助医生护士全面的掌握患者病情，实现对AVF术后患者的系统化管理和个性化护理，提高医疗护理质量，完成随访数据库的建立以供临床科研和教学进一步使用。同时也能够满足患者更高层次的医疗护理需求，为其他延续性护理工作提供借鉴和参考。  

Objectives of Study：

At present, the post-operative nursing mode of autologous arteriovenous fistula (AVF) in China mainly focuses on routine nursing intervention in hospital. However, due to the lack of nursing knowledge, weak self-management awareness, poor compliance of outpatient follow-up and other factors, the patients may have poor maturity of AVF and AVF related complications, thus affecting the smooth progress of hemodialysis treatment. This study hopes to help doctors and nurses comprehensively grasp the patient's condition, realize the systematic management and personalized care of patients after AVF, improve the quality of medical care, and complete the establishment of follow-up database for further use in clinical research and teaching through the application of the vascular access outpatient service of medical care integration in the continuous care after AVF. At the same time, it can also meet the higher-level medical care needs of patients and provide reference for other continuous nursing work.

纳入标准：

纳入标准：①自愿参与本研究，意识清晰，认知正常；②首次AVF手术且手术成功；③无其他重大器官或组织损伤；④会使用微信。

Inclusion criteria

Inclusion criteria: ① voluntary participation in the study, clear consciousness and normal cognition; ② The first AVF operation was successful; ③ No other major organ or tissue damage; ④ Will use wechat.

排除标准：

排除标准：①有严重心脑血管疾病；②合并自身免疫性、感染性疾病；③合并系统性皮肤疾病；④合并恶性肿瘤；⑤沟通障碍，无法配合研究。

Exclusion criteria：

Exclusion criteria: ① serious cardiovascular and cerebrovascular diseases; ② Combined with autoimmune and infectious diseases; ③ Complicated with systemic skin diseases; ④ Combined with malignant tumor; ⑤ Communication barrier, unable to cooperate with the research.
