研究目的：

1. 主要目的 评价重组金黄色葡萄球菌疫苗（大肠杆菌）对18～70周岁骨科手术目标人群术后预防金黄色葡萄球菌感染的保护效力。 2. 次要目的 （1）评价重组金黄色葡萄球菌疫苗（大肠杆菌）在18～70周岁骨科手术目标人群中的安全性。 （2）评价重组金黄色葡萄球菌疫苗（大肠杆菌）在18～70周岁骨科手术目标人群中的免疫原性。  

Objectives of Study：

1.Primary objective To evaluate the protective efficacy of recombinant Staphylococcus aureus vaccine (E. coli) against Staphylococcus aureus infection in the target population aged 18-70 years undergoing orthopaedic surgery. 2. Secondary objectives (1) To evaluate the safety of recombinant Staphylococcus aureus vaccine (E. coli) in the target population aged 18-70 years undergoing orthopaedic surgery. (2) To evaluate the immunogenicity of recombinant Staphylococcus aureus vaccine (E. coli) in the target population aged 18-70 years undergoing orthopaedic surgery.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.18周岁≤年龄≤70周岁，男女不限；
2.跟骨、pilon骨折、胫骨干、胫骨平台、股骨远端等闭合性单一或多发性骨折择期手术患者；
3.育龄期女性或男性受试者须同意在试验期间采取高效的避孕措施；
4.自愿参加本研究，并签署知情同意书。

Inclusion criteria

1. Males or females aged ≥ 18 years and ≤ 70 years;
2. Patients undergoing elective surgery for closed single or multiple fractures of the calcaneus, pilon, tibial shaft, tibial plateau, distal femur, etc.;
3. Women of childbearing potential or male subjects must agree to take effective contraceptive measures during the trial;
4. Subjects who voluntarily participate in this study and sign an informed consent form.

排除标准：

1. 签署ICF前12个月内有金黄色葡萄球菌感染病史；
2. 内固定术前外固定物未移除且将持续使用≥2周或外固定术后发生针道感染，或经研究者评估内固定术后还需进行外固定者；
3. 首次免疫前3个月内接受过血液制品或免疫球蛋白；
4. 有试验疫苗成份（含mHIN2、SpA5、mSEB、MntC蛋白及磷酸铝）过敏史，或有任何疫苗接种严重过敏史（如急性过敏反应、哮喘、荨麻疹、呼吸困难、血管神经性水肿或腹痛等）；
5.根据病史已知任何确诊的或可疑的免疫抑制或免疫缺陷状态，包括人免疫缺陷病毒感染；或有器官移植史；
6.有甲状腺切除史，或在筛选前6个月内由于甲状腺疾病需要治疗；
7.无脾，功能性无脾，以及任何情况导致的无脾或脾切除；
8.有凝血因子缺陷或凝血功能障碍导致的出血性疾病史，或筛选时因凝血相关疾病需持续使用抗凝药；
9.签署ICF前6个月内长期使用免疫抑制剂治疗（超过14天）、细胞毒性治疗、吸入皮质类固醇（不包括过敏性鼻炎的皮质类固醇喷雾治疗，急性非并发皮炎的表面皮质类固醇治疗）；
10.首次免疫前28天内接种过减毒活疫苗；
11.首次免疫前14天内接种过任何其他疫苗或抗过敏治疗；
12.拟注射部位上臂三角肌皮肤异常者；
13.存在血糖控制不佳（空腹静脉血糖≥11mmol/L）的糖尿病、活动性严重感染、血管性水肿、严重心脑血管疾病、严重呼吸系统疾病（如支气管哮喘、活动性结核、肺纤维化）、恶性肿瘤病史（已痊愈超过5年的皮肤基底细胞癌或子宫颈原位癌除外）以及其他严重疾病，经研究者判定参加试验可能增加疾病风险或干扰本临床试验观察者；
14.有晕针、晕血史，或不能耐受静脉穿刺采血的患者；
15.可疑或已知的酒精或药物依赖；
16.妊娠或哺乳期女性，或研究期间有妊娠或哺乳计划的受试者；
17.签署ICF前3个月内参加过其他药物或医疗器械临床试验；
18.有严重神经或精神疾病病史及其他无行为能力或认知能力者；
19.研究者认为不适合参加本试验的其他情况。

Exclusion criteria：

1. Medical history of Staphylococcus aureus infection within 12 months before signing the informed consent form;
2. The external fixator is not removed before internal fixation but will be continued for ≥2 weeks or pin-site infections occur after external fixation, or external fixation is considered to be necessary by the investigator after internal fixation;
3. Use of blood products or immunoglobulins within 3 months before the first dose;
4. Allergic to components (including mHIN2, SpA5, mSEB, MntC protein and aluminum phosphate) of the investigational vaccine, or with a history of severe allergic reactions to vaccination (e.g., acute allergic reaction, asthma, urticaria, dyspnea, angioedema or abdominal pain, etc.);
5. Known to be immunosuppressed or immunodeficient that has been confirmed or suspected based on medical history, including human immunodeficiency virus infection, or a history of organ transplantation;
6. Subjects with a history of thyroidectomy, or requiring treatment due to thyroid disease within 6 months before screening;
7. Presence of asplenia, functional asplenia, and asplenia or splenectomy under any circumstances;
8. Medical history of bleeding disorders due to coagulation factor deficiency or coagulation disorder, or anticoagulants required to be continued due to coagulation-related diseases during screening;
9. Long-term use of immunosuppressive therapy (more than 14 days), cytotoxic therapy, inhaled corticosteroids (excluding corticosteroid spray therapy for allergic rhinitis, and topical corticosteroid therapy for acute non-complicated dermatitis) within 6 months before signing the informed consent form;
10. Use of any live attenuated vaccine within 28 days before the first dose;
11. Use of any other vaccine or anti-allergy treatment within 14 days before the first dose;
12. Presence of skin abnormality at the deltoid muscle of the upper arm where it is intended for injection;
13. Presence of diabetes with suboptimal glycemic control (fasting venous blood glucose ≥11 mmol/L), active severe infection, angioedema, severe cardiovascular and cerebrovascular diseases, severe respiratory diseases (e.g., bronchial asthma, active tuberculosis, pulmonary fibrosis), a history of malignancy (subjects who have basal cell carcinoma of the skin or carcinoma in situ of the cervix cured for more than 5 years are acceptable), or other serious diseases, or participation in the trial that at the discretion of the investigator may increase the risk of disease or interfere with the observation of this clinical trial;
14. Patients with a history of fainting at the sight of a needle or blood, or unable to tolerate phlebotomy;
15. Subjects who are suspected or known to have alcohol or drug dependence;
16. Pregnant or breastfeeding women, or subjects who plan to become pregnant or breastfeed during the study;
17. Participation in clinical trials of other drugs or medical devices within 3 months before signing the informed consent form;
18. Presence of medical history of severe neurological or psychiatric diseases, and those without capacity to act or cognitive ability;
19. Other conditions that are deemed inappropriate by the investigator to participate in this trial.
