研究目的：

在全国范围内选取11家有一定科研实力、在心血管疾病管理和血糖管理方面均有一定学术基础，并且心内科门诊量较大的医院作为试点，在未诊断糖尿病的心血管内科门诊患者中进行规范血糖筛查，以了解心内科门诊患者糖代谢异常发病率的流行病学调查。  

Objectives of Study：

Selected 11 hospitals with large outpatients of Department of Cardiology across the country as a pilot, and these hospitals should have a certain research strength and academic foundation in cardiovascular disease management and blood glucose management, then conduct regulate blood sugar screening in those undiagnosed diabetes cardiovascular patients, in order to investigate the abnormal glucose metabolism incidence of Cardiology outpatients.

纳入标准：

纳入标准：
1. 男或女性,年龄 ≥ 18岁
2. 有明确的心血管疾病史，符合以下条件者：
1） 冠心病患者（冠心病诊断标准参见2010年中华人民共和国卫生部 冠状动脉粥样硬化性心脏病诊断标准）
a) 新诊断冠状动脉粥样硬化性心脏病;
b) 既往冠状动脉粥样硬化性心脏病史;
c) 既往心肌梗死史;
2） 高血压合并以下一种或一种以上其他糖尿病危险因素
a) 糖尿病家族史；
b) 超重或肥胖，BMI≥ 24 kg/m2；
c) 血脂异常（血脂异常定义参见附录2）；
d) 既往生产过巨大儿（体重≥4kg）或妊娠期糖尿病史；
e) 年龄≥ 45岁；

Inclusion criteria

1. Male or female, age >=18 years;
2. Have a determinate history of cardiovascular disease, meet the following conditions:
(1) Coronary heart disease (CHD diagnostic criteria refer to the China Ministry of Health in 2010 - coronary heart disease diagnostic criteria)
Newly diagnosed coronary heart disease;
Previous coronary heart disease;
Previous myocardial infarction;
(2) Hypertension complicated with one or more the following diabetes risk factors:
Family history of diabetes;
Overweight or obese, BMI >=24 kg/m2;
Dyslipidemia (dyslipidemia definition see Appendix 2);
Women who delivered a baby weighing >=4kg or were diagnosed with GDM;
Age >=45 years.

排除标准：

有以下情况之一不能纳入本研究：
1. 既往有糖尿病病史，妊娠糖尿病病史除外；
2. 现正口服降糖药；
3. 最近一个月内发生过心肌梗死（MI）、脑中风或短暂性脑缺血发作（TIA）；
4. NYHA分级Ⅲ或Ⅳ级的心力衰竭；
5. 计划近期进行血运重建手术或其他类型手术治疗；
6. 患有严重的肝脏疾病；
7. 患有严重的肾功能损害（GFR <25 ml/min/1.73m2)；
8. 已妊娠或在随后1年内计划妊娠；
9. 研究者认为不适合参与此项临床试验；

Exclusion criteria：

Patients who have one of the following conditions should be exclude from the study:
1. Previous diabetes, except previous gestational diabetes;
2. Currently taking oral antihyperglycemic agents;
3. Have episode of myocardial infarction (MI), stroke or transient ischemic attack (TIA) for the past month;
4. NYHA class Ⅲ or Ⅳ heart failure;
5. Planned revascularization surgery or other types of surgery recently;
6. With severe liver disease;
7. With severe renal impairment (GFR <25 ml/min/1.73m2);
8. Been pregnant or planning pregnancy in the subsequent year;
9. The researchers believe that the patient was not suitable for this
